Kosten-Nutzen-Analyse der Malariaprophylaxe bei Keniareisenden mit Mefloquin

Zusammenfassung

In Zeiten einer zunehmenden Ökonomisierung des Gesundheitswesens müssen sich auch präventivmedizinische Massnahmen an ihrem ökonomischen Nutzen messen lassen. Dass Prävention sich auszahlen kann, dass sie sich gleichsam selbst zu finanzieren in der Lage ist, belegen die Ergebnisse einer Kosten-Nutzen-Analyse der Chemoprophylaxe der Malaria tropica mit Mefloquin bei Keniareisenden. Für das auf Basis einer befragungsgestützten Untersuchung (Malpro-Studie) analysierte Reisendenkollektiv liegen die bei Durchführung der Mefloquin-Prophylaxe induzierten monetären Folgewirkungen sowohl für die Schweiz als auch für die Bundesrepublik Deutschland unter jenen der Nicht-Durchführung. Die Prophylaxe kompensiert damit nicht nur ihre Kosten, sondern bewirkt damit zudem einen gesamtwirtschaftlichen Nutzenüberschuss. Ökonomisch begründete Widerstände gegen die Mefloquin-Prophylaxe der Malaria bei Kurzzeitaufenthaltern in Hochrisikoländern sind damit nicht gerechtfertigt.

Résumé

A l'heure où la volonté d'économiser est croissante dans le domaine de la santé, la médecine préventive se doit elle aussi être évaluée sur le plan économique. La prévention est rentable et elle est en mesure de s'autofinancer: c'est ce que montrent les résultats d'une analyse coûts/bénéfice de la chimioprophylaxie du paludisme à P. Falciparum par la Méfloquine chez des voyageurs se rendant au Kenya. Dans un collectif de voyageurs analysé dans une étude par questionnaire (étude Malpro), les répercussions financières de la prophylaxie par la Méfloquine sont plus faibles, tant en Suisse qu'en République fédérale d'Allemagne, que celles entraînées par l'absence de prophylaxie. Il en résulte que non seulement la prophylaxie compense son coût, mais aussi qu'elle dégage un excédent «bénéficiaire» sur un plan économique global. Il apparaît ainsi que les arguments économiques avancés à l'encontre d'une prophylaxie du paludisme par la Méfloquine lors de brefs séjours dans des régions à haut risque ne sont pas justifiés.

Summary

With an increasing movement towards cost saving in the health sector, preventive medicine must also be judged according to its economic viability. The fact that prevention can autofinance itself is suggested by the results of a cost/benefit analysis of chemoprophylaxis of Faiciparum malaria with Mefloquin among travellers in Kenya. Out of the whole group of travellers analysed by means of an interview-based test (Malpro-Study), the costs in the case of both Switzerland and the Federal German Republic were lower for those people who had undergone Mefloquin-prophylaxis than for those who had not. In this way the prophylaxis not only compensates the required outlay but also results in an overall benefit in macroeconomic terms. Therefore economically based opposition to the prophylaxis of malaria with Mefloquin for short stays in high-risk countries is not justified.
