Zusammenfassung
Die Arbeit untersucht den Einfluss sozialer Faktoren auf das Geburtsgewicht und die vor- und nachgeburtliche Sterblichkeit in der schweizerischen Geburtenkohorte von 1979–85 (N=519 933). Der Anteil untergewichtiger Neugeborener (<2500 g) steigt mit sinkender Sozialklasse. Die Totgeburtenrate, die neonatale und postneonatale Mortalität sind in den unteren Sozialklassen deutlich höher als in den oberen; eine Geburtsgewichtsstandardisierung schwächt zwar den Mortalitätsunterschied zwischen den Sozialklassen ab, ohne ihn aber zum Verschwinden zu bringen. Grössere soziale Unterschiede in der perinatalen Sterblichkeit werden bei den normalgewichtigen Neugeborenen beobachtet,sie sind hingegen kaum bei den Untergewichtigen anzutreffen.
Résumé
L'influence des facteurs sociaux sur le poids à la naissance des nouveaux-nés ainsi que sur la mortalité foetale et infantile a été étudiée, en Suisse, sur la cohorte de nouveaux-nés durant la période couvrant les années 1979–1985 (N=519 933). La proportion des nouveaux-nés avec un petit poids à la naissance (<2500 grs) était plus élevée dans les classes sociales les moins favorisées. La même constatation s'est imposée en égard aux mortalités néonatales, postnatales ainsi qu'au taux des morts-nés. Toutefois, la comparaison des taux de mortalité après contrôle selon le poids à la naissance rend cette constatation moins percutante. En effet, alors que l'influence des facteurs sociaux sur la mortalité périnatale est indiscutable dans le groupe des nouveaux-nés présentant un poids normal à la naissance, celle-ci s'estompe lorsque l'on considère le groupe des nouveaux-nés avec un petit poids à la naissance.
Summary
The influence of social factors on birthweight and fetal and infant mortality was investigated in the Swiss birth cohort from 1979–85 (N=519 933). The proportion of newborns with low-birthweight (<2500 g) was higher in lower social classes. Stillbirth-rate, neonatal and postneonatal mortality were higher in lower social classes, too. When controlling for birthweight, the increase in mortality in the lower social classes became somewhat less striking. Marked social differences in perinatal mortality were found in the newborns with normal weight, whereas almost no difference could be detected in the low-birthweight-group.
