Zusammenfassung
Die Problematik der ungleichen Risikoverteilung in den Krankenkassen rückt zunehmend in das Blickfeld der Reformdiskussion zur Gesetzlichen Krankenversicherung in der BRD. Der folgende Beitrag stellt die Frage nach den strukturellen Ursachen von Risikosegregation und verweist dabei auf die Zusammenhänge von sozialer Schichtung. Erkrankungsrisiko und Versicherungsstatus. Ein systemübergreifendes Modell der grundlegenden Bedingungen der Risikosegregation wird vorgestellt und die jeweiligen Zusammenhänge anhand von deutschen und amerikanischen Befragungsdaten empirisch überprüft. Die Ergebnisse der statistischen Analysen bestätigen die postulierten Zusammenhänge und veranschaulichen die systemübergreifende Bedeutung sozialstruktureller Faktoren für die Risikosegregation. Der Beitrag schliesst mit einigen Anmerkungen zur Relevanz dieser Ergebnisse sowohl für die Diskussion zur anstehenden Strukturreform der gesetzlichen Krankenversicherung als auch für zukünftige Forschungsaufgaben.
Resumé
Le problème posé par la distribution inégale des risques dans les les caisses-maladie so pose de plus en plus en Allemagne Fédérale, comme dans d'autres pays. Cet article examine les causes structurelles de la ségrégation de risques en analysant les associations entre couche sociale, risque de maladie et statut d'assurance. Sur la base d'une enquête comparative entre l'Allemagne et les Etats-Unis, un modèle d'analyse est proposé. En principe, les résultats confirment ce modèle, et ils démontrent, en Allemagne et aux Etats-Unis, l'importance des influences socio-structurelles sur la ségrégation. En conclusion, signification des résultats obtenus pour la réforme actuelle des caisses-maladie et pour la recherche scientifique est examiniée.
Summary
Issues of unequal risk distribution among sickness funds are given increasing attention in the current discussions on the reform of the statutory health insurances system in Germany. This paper raises the question on the structural determinants of risk segregation and points towards the links between social stratifacation, health risk and insurance status. A model showing the links between basic structural determinants is presented. Using health survey data from Germany and the U.S. statistical analyses are conducted. The results support the model and indicate ist applicability for both health care systems. The paper concludes by indicating the relevance of such findings for health policy and future research.
