Summary
Maintaining the integrity of the abdominal cavity must be the goal of surgical measures performed in the therapy of postoperative peritonitis. Antibiotics, lavage, and drainage are important supportive measures of varying value. Although no conflicting opinions exist with regard to a parenteral therapy with antibiotics, their local application no longer seems warranted now that antiseptic agents provide an effective alternative. Intraoperative lavage in cases of diffuse peritonitis is generally accepted, and continuous postoperative lavage is, given the necessary experience, practicable. The use of drainage, which is certainly a sensible procedure for local peritonitis, is of limited effectiveness in the treatment of diffuse peritonitis.
Zusammenfassung
Die Integrität der Bauchhöhle muß das Ziel der chirurgischen Maßnahmen in der Behandlung der postoperativen Peritonitis sein. Antibiotica, Spülung und Drainage sind wichtige flankierende Maßnahmen von unterschiedlichem Stellenwert. Bestehen hinsichtlich einer parenteralen Antibioticatherapie keine kontroversen Auffassungen, so erscheint die lokale Anwendung nicht mehr vertretbar, seitdem mit Antiseptica wirksame Alternativen zur Verfügung stehen. Die intraoperative Lavage bei diffuser Peritonitis ist allgemein akzeptiert, die postoperative Dauerspülung bei entsprechender Erfahrung praktikabel. Drainagen, bei lokaler Peritonitis sicher sinnvoll, bleiben für die diffuse Peritonitis von begrenzter Wirksamkeit.
