Summary
Severe hyperhidrosis has serious effects on the private and professional life of the patients affected and makes many professions impossible for them. Preoperative investigations-transpiration test, electrothermogram, sympathicogalvanogram-show the extent of the functional peripheral circulatory disorder.
For the upper extremity a sympathicotomy at the sudo- and vasomotor fibres of the thoracal sympathetic ganglions (rami communicantes Th II to Th IV) is performed by means of endoscopy. The sympathetic ganglions are easily visible under the thin parietal pleura. For the lower extremity the endoscopic approach has become possible with development of the new method of retroperitoneoscopy. Here the endoscope is put in position and the retroperitoneal cavity is inflated with CO2 until the lumbal ganglions, on the right side between the psoas and V. cava and on the left between the psoas and aorta, are easily accessible. Dissection is performed for the foot and tibial region at L IV, and for the femoral region at L II/III, depending on the indication. Postoperative tests verify the results of sympathicotomy. Zusammenfassung
Die schwere Hyperhidrosis wirkt sich stark auf die privaten und beruflichen Umweltbeziehungen aus, macht die Ausübung zahlreicher Berufe unmöglich. Voruntersuchungen — Schweißtest, Elektrothermogramm, Sympathicogalvanogramm —zeigen das Ausmaß der funktionellen peripheren Durchblutungsstörung. Für die obere Extremität wird eine Sympathicotomie an den sudo- und vasomotorischen Fasern des thorakalen Grenzstrangs — rami communicantes Th II bis IV — auf endoskopischem Wege durchgeführt. Der Grenzstrang ist unter der dünnen Pleura parietalis gut sichtbar.
Für die untere Extremität ist der endoskopische Zugang durch die neue Methode der Retroperitoneoskopie möglich geworden, bei der der Retroperitonealraum unter CO2-Füllung mit dem Endoskop angelegt und so weit erweitert wird, daß der lumbale Grenzstrang rechts zwischen Psoas und Cava, links zwischen Psoas und Aorta gut zugänglich ist. Hier wird die Ausschaltung je nach der Indikationsstellung für den Fuß-Unterschenkelbereich bei L IV, für den Oberschenkel bei L II/III durchgeführt.
Postoperative Tests zeigen das Ergebnis der Sympathicusausschaltungen. Film 16 mm, 15 min, Farbe, Magnetton.
