54. Vorbehandlung, operative Technik, Nachbehandlung des toxischen Adenoms, bzw. hyperthyreoten Struma

Summary

The “hot adenoma” has a special position among the hyperthyroid forms of goiter, since only one isolated section of the thyroid gland is hormonally over-active. The main clinical symptoms are tachyarrythmia, loss of weight and nervousness, while there are no exophthalmic symptoms. The diagnosis depends completely on the localization test and the two-phase test with 1311, which help in differentiation of a decompensated from a compensated toxic adenoma. The preferred therapy is isolated enucleation of the adenoma. It has proved advantageous to prepare the patients for surgery with thyrostatics and Lugols solution. Six to eight weeks after the operation another two-phase test with¹³¹I must be performed to allow assessment of the appropriate follow-up treatment to prevent recurrences.

Zusammenfassung

Das toxische Adenom nimmt unter den hyperthyreoten Strumen eine Sonderstellung ein. Ein isolierter Bezirk der Schilddrüse ist hormonell überaktiv. Hauptsymptome sind vor allem Tachyarrhythmien, Gewichtsabnahme und Nervosität, während Augensymptome (Exophthalmus) fehlen. Die Diagnose stützt sich allein auf den Radiojod-Test. Wir unterscheiden ein dekompensiertes und kompensiertes toxisches Adenom. Die bevorzugte Therapie ist die isolierte Enucleation des Adenoma. Als vorteilhaft hat sich die päoperative Vorbereitung der Patienten mit Thyreostatica und Lugolscher Lösung erwiesen. 6-8 Wochen nach der Operation muß ein neuerlicher Radiojod-Test durchgeführt werden, um eine entsprechende Rezidivprophylaxe ausschließen zu können. Bisher haben wir keine Rezidive beobachtet.
