Operative principles in primary parathyroid hyperplasia and in reoperations

Summary

In recent years intraoperative decision making in surgery for primary HPT became standardized. Difficulties still arise in hyperplasia and in reoperations. A review of the literature shows that the standard 3 1/2-resection is still the procedure of choice in sporadic hyperplasia. In familial disease or MEN-syndromes total parathyroidectomy and autotransplantation seem to be more advantageous. Reoperations require a delicate operative technique. Cryopreservation of the excised parathyroid tissue is necessary because of a high incidence of permanent postoperative hypoparathyroidism.

Zusammenfassung

Die Zunahme der Eingriffe wegen primären Hyperparathyreoidismus (HPT) hat u. a. zu einer Standardisierung der operativen Behandlung geführt. Problematisch ist noch das Vorgehen bei der primären Hyperplasic und bei Reoperationen. Daten aus der Literatur weisen darauf hin, daß die subtotale (3 1/2) Resektion nach wie vor als Standard-behandlung für die primäre Vierdrüsenhyperplasie gelten muß. Bei familiärer Erkrankung und dem HPT im Rahmen der MEN-Syndrome erscheint die totale Parathyreoidektomie und Autotransplantation als das bessere Verfahren. Reoperationen stellen hohe Anforderungen an die operative Technik. Wichtig ist es, einen Teil des entnommenen Gewebes zu konservieren, da mit einem hohen Prozentsatz von permanentem Hypoparathyreoidismus zu rechnen ist.
