Summary
Certain cases of maβive pulmonary embolism have such a rapid course that no time is left for operation in extracorporeal circulation, and that even the claβical Trendelenburg operation is not succeβful because of lack of time or exceβive cardiac streβ. This is especially valid for patients where heart arrest has already occured. In such extreme emergencies the left sided thoracotomy and embolectomy through the left pulmonary artery is both the fastest and the least traumatic method. Three patients were operated and two survived.
Zusammenfassung
Bei einem Teil der Patienten mit maβiver Lungenembolie ist der fatale Verlauf so rasch, daß keine Zeit zur Operation mit extrakorporaler Zirkulation verbleibt und auch die klaβische Trendelenburgsche Opera tion zu spät kommt. Dies gilt besonders für Fälle mit bereits eingetrete-nem Herzstillstand. In solchen extremen Notfällen ist die linkβeitige Thorakotomie und Embolektomie durch die linke Pulmonalarterie zugleich am schnellsten und schonendsten. Die chirurgische Technik wird dargestellt. Drei Patienten wurden operiert, zwei überlebten.
