Summary

Acute complete thrombosis of the terminal aorta leads to a pain symptomatology with absence of pulse, paraplegia, shock, and gangrene. Renal failure and acute abdomen may develop. Thrombosis mostly develops with aortic occlusion disease, thromboembolism, trauma, thrombotic or dissecting aneurysm, and infection. Of the 14 cases reported, 4 died without surgical therapy, 10 underwent surgery (5 thrombectomies, 3 aortobifemoral bypasses, 1 laparotomy and 1 axiofemoral bypass), and 7 died after surgery. An axillo-femoral bypass is used if thrombectomy fails to restore normal pulsatide flow.

Zusammenfassung

Die akute komplette Thrombosierung führt zur plötzlichen Schmerzsymptomatik: Pulslosigkeit, Paraplegie, Schock, Gangrän, Nierenversagen und akutes Abdomen können vorkommen. Ursache ist chronische AVK, Thrombembolien, Trauma, thrombotische bzw. dissektierende Aneurysma und Infektionen. Zusätzlich bestehen schwerwiegende Vorerkrankungen. Berichtet wird über 14 Patienten; 4 Patienten verstarben; 10 Patienten wurden operiert. 5mal Thrombektomie, 3mal aorto-bifemoraler Bypass, je 1mal Laparotomie und axillo-femoraler Bypass. 7 Patienten verstarben nach chirurgischer Intervention. Bei Versagen einer Thrombektomie — Anlage eines axillo-femoralen Bypasses.
