6. Die „offenen” spinalen Schmerzoperationen (anterolaterale Chordotomie und kommissurale Myelotomie) in der modernen Schmerzbekämpfung

Summary

The only significant open pain operations on the spinal cord itself that are routinely in use nowadays are anterolateral chordotomy and commissural myelotomy. Open thoracic chordotomy is still indicated in cases of malignant growth in the lower body, to be sure of avoiding further neurological involvement in the upper region. Open cervical chordotomy, on the other hand, is now only indicated after unsuccessful percutaneous chordotomy and neurostimulation technics. Commissural myelotomy is still used in specific cases in specialized clinics. Despite advances in knowledge and more modern methods, the open operations still cannot be abandoned.

Zusammenfassung

Von den „offenen” Schmerzoperationen am Rückenmark haben heute nur noch die anterolaterale Chordotomie und die kommissurale Myelotomie als Routinemaßnahmen Bedeutung. Die offene thorakale Chordotomie ist indiziert bei malignen Prozessen der unteren Körperhälfte, wenn zusätzliche neurologische Störungen in der oberen Körperhälfte mit Sicherheit vermieden werden sollen. Die offene zervikale Chordotomie wird heutzutage in der Regel nur noch bei Versagen der percutanen Chordotomie oder der Neurostimulationsverfahren zur Anwendung kommen. Die kommissurale Myelotomie bleibt besonderen Fällen in Spezialkliniken vorbehalten. Trotz neuerer Erkenntnisse und modernerer Methoden kann aber auf die „offenen” Eingriffe noch immer nicht verzichtet werden.
