Complications in sexual behavior after rectal surgery

Summary

Disturbances in male fertility are the most frequent, but less regarded complications after resection or amputation of the rectum. They are caused by intra-operative lesions of the autonome lumbo-sacral nerves. In addition, psychic factors play part in people bearing a colostomy, especially in women. After amputation of the rectum 49 % of the male patients (n = 74) noticed an impotentia coeundi or generandi. A loss of penis-erection was marked by 44 %, while 37 % were unable to ejaculate. These figures show, that the defect commonly is complete. After resection of the rectum, there were 19 % with an impossibility of penis-erection, 7 % without ejaculation. Only in few cases function normalized later on. In incomplete lesions therapy was successfully made by giving an androgen-substitution, in case combined with thymoleptica.

Zusammenfassung

Störungen der Potenz sind die häufigsten, gleichwohl am wenigsten beachteten Komplikationen nach skeletierenden Rectumoperationen. Sie rind Folge intraoperativer Läsionen autonomer lumbo-sacraler Nerven. Zusätz-lich spielen psychische Faktoren bei Kunstaftertägern eine Rolle. Nach Rectumamputationen wiesen 49 % eine Impotentia coeundi oder generandi auf. Ein Verlust der Erektionsfähigkeit bestand bei 44 %, ein Ausfall der Ejaculation bei 37 %. Nach Rectumresektion trat bei 19 % eine Impotentia coeundi, bei 7 % eine Ejaculations-störung auf. Bei inkompletten Veränderungen ist nach urologischer Abklärung ein Therapieversuch mit einem Androgenpräparat, gegebenenfalls in Verbindung mit einem Thymolepticum, empfehlenswert.
