Zusammenfassung
Bei der wei\en Ratte führt eine überdosierung vonCortisol zu einem Abbau des Fettgewebes. Dabei kommt es im Blut zu einem geringen Anstieg von K, Na und Cl, in Herzmuskel und Skeletmuskel zu einem starken Anstieg des K sowie auch des Na und Cl, in Nieren und Lungen zu einem Verlust von K und einem Anstieg von Na und Cl. An den Nieren entwickelt sich eine nekrotisierende Nephrose besonders im proximalen Nephron, am Herzen eine Druckhypertrophie mit Nekrose vereinzelter Herzmuskelzellen. Elektronenmikroskopisch zeigt der Herzmuskel VerÄnderungen an denMitochondrien, die für eine osmoregulatorische AktivitÄt dieser Zellorganellen sprechen. Die Herzmuskelzelle ist offenbar in der Lage, die vorherrschende katabole Stoffwechsellage zu durchbrechen, so da\ es zur Hypertrophie kommen kann.
Nach einer überdosierung vonAldosteron kommt es schon im akuten Versuch zu einem Gewichtsanstieg durch Wassereinlagerung bei gleichzeitigem Verlust von K und Anstieg von Na und Cl in allen Organen. Die submikroskopischen VerÄnderungen des Herzmuskels Ähneln denen nach überdosierung von Cortisol, sind aber stÄrker ausgeprÄgt.
Eine chronische Belastung mitNatriumphosphat führt zu einer Inanition. An den Nieren kommt es zu einer interstitiellen Nephritis, Nephrocalcinose und Nephrohydrose mit dem Endzustand einer UrÄmie. Am Herzen findet man eine mÄ\ige Atrophie, GefÄ\verkalkungen, Mikrothrombosen und herdförmige Muskelnekrosen.
Eine spezifische kardiotoxische Wirkung der Corticosteroide lÄ\t sich nicht nachweisen. Die sog. Phosphat-Steroid-Cardiopathie resultiert aus dem Zusammentreffen zweier grundsÄtzlich verschiedener pathogenetischer Prozesse.
Summary
An overdose ofhydrocortisone in the albino rat leads to a break-down of fat tissues. In addition, there is a slight increase of K, Na, and Cl in the blood: in the cardiac and skeletal muscles the K, Na, and Cl sharply increase. In the kidneys and lungs there is a loss of K, but the Na and Cl increase. A necrotizing nephrosis develops particularly in the proximal nephron. In the heart, hypertrophy due to hypertension occurs, with vacuolization or necrosis of individual muscle cells. Electronmicroscopically the mitochondria of the cardiac muscle show changes which suggest these cell organelles possess an osmoregulatory activity. The myocardial cell apparently is able to overcome the predominating catabolism, thus giving rise to cardiac hypertrophy.
After an overdose ofaldosterone in short term experiments, there is an increase in weight due to an uptake of water with a simultaneous loss of K and an increase of Na and Cl in all organs. The submicroscopic changes in the cardiac muscle resemble those after an overdose of hydrocortisone; they are, however, more marked.
A chronic overloading withsodium phosphate leads to an inanition. An interstitial nephritis develops, with nephrocalcinosis and nephrohydrosis, with uremia as the end-stage. In the heart one finds moderate atrophy, calcification of vessels, microthrombi, and focal necroses of muscle. A specific cardiotoxic effect of the corticosteroids could not be demonstrated. The socalled phosphate-steroid-cardiopathy results from the dual action of two basically different pathogenetic processes.
