Zusammenfassung
Bei 79 Kindern wurden histologisch die Veränderungen in der Haut nach BCG-Vaccination verfolgt. Die Frühreaktion ist rein leukocytär. Ein spezifisches Granulom bildet sich etwa 3 Wochen nach der Impfung. Das floride spezifische Granulationsgewebe überdauert bis zum 4. Monat, später überwiegt eine Vermehrung kollagener Fasern, so daß in manchen Fällen schon vom 8. Monat an eine Narbe entwickelt ist, die sonst regelmäßig erst nach 4 Jahren vorhanden ist.
Vom gewöhnlichen tuberkulösen Granulationsgewebe unterscheiden sich die Veränderungen nach BCG-Vaccination durch häufiges Vorhandensein von Schaumann-Körpern und nichtlipoiden Kristallen (wohl Calciumcarbonat). Die Schaumannschen Körperchen werden mit dem Zerfall von Fettzellen des Unterhautfettgewebes bzw. Fettsäurekristallen sowie mit nichtlipoiden Kristallen in Zusammenhang gebracht.
Tuberkelbacillen fanden sich am Inoculationsort 17mal und konnten bis zum 5. Monat nach der BCG-Vaccination nachgewiesen werden. Spezifische Granulome in den axillären Lymphknoten fanden sich 10mal. und zwar vom 2. bis zum 8. Monat nach der Impfung in 46 daraufhin untersuchten Fällen.
Summary
The cutaneous changes occurring after the vaccination with BCG were followed histologically in 79 children. The early reaction was characterized by acute inflammatory cells. A specific granuloma formed about three weeks after the inoculation. The exuberant granulomatous tissue persisted up to four months; later collagen fibers predominated, thus in many instances by the eighth month a scar had developed, which generally was first evident after four years.
The changes after BCG vaccination were distinguished from the usual tuberculous granulation tissue by the frequent Schaumann bodies and non-lipoid crystals (calcium carbonate). The Schaumann bodies were related to the destruction of fat cells of the subcutaneous adipose tissue, that is, to the fatty acid crystals as well as to non-lipoid crystals.
Tubercle bacilli were found at the site of inoculation 17 times, and could be demonstrated up to the fifth month after the BCG-vaccination. Specific granulomata were found in the axillary lymph nodes 10 times, from the second to the eighth month after the inoculation in 46 of the cases studied.
