Zusammenfassung
In der Niere von 10 von 12 Patienten, die kurz vor dem Tode einen Schock erlitten hatten bzw. an Schockläsionen der Niere verstorben waren, wurde eine Hypertrophie, Hyperplasie und Hypergranulation der granulierten Zellen des juxtaglomerulären Apparates beobachtet. Die Frequenz der granulierten Zellen schwankt zwischen 10,3 und 29,2%, mit einem Durchschnitt von 20,0%; gegenüber 1% in Kontrollen. In allen Fällen, in denen der Serum-Na-Wert bestimmt wurde, konnte eine Hyponaträmie festgestellt werden. In einem Fall von längerer Krankheitsdauer mit Rückkehr des Serum-Na-Wertes zur Norm einige Tage vor dem Tod sowie in der Niere eines bereits längere Zeit an Hypertonie leidenden Patienten war eine annähernd normale Frequenz festzustellen. Die Frequenzzunahme wird auf die mit dem Schock einhergehende Hypotonie bzw. Ischämie der Niere, sowie auf die Erniedrigung des Serum-Na-Spiegels zurückgeführt und als kompensatorisches Phänomen aufgefaßt.
Summary
In 10 of 12 patients, who shortly before death had experienced shock or had died because of shock-induced renal lesions, hypertrophy, hyperplasia, and a hypergranulation of the granular cells of the juxtaglomerular apparatus were observed. The frequency of granular cells varied between 10.3% and 29.2% with an average of 20%, in contrast to 1% of the controls. In all the patients in whom the serum sodium had been determined, there was a hyponatremia. Almost normal numbers of juxtaglomerular cells were detected in one patient with a prolonged illness and return of the serum sodium to normal some days before death, and in another patient who had suffered from hypertension for a long time. The increase in frequency was related to the hypotension or ischemia of the kidney, as well as to the lowering of the serum level of sodium, and was interpreted as a compensatory phenomenon.
