For the purposes of the present discussion, the term “provider” refers only to the dentist. While it is recognized that dental hygienists, nurses, therapists, and other assistants provide services, typically they do so under the supervision of a dentist, who continues to be the main provider in the United States. After this paper was presented, a report by DHHS indicated that auxiliaries would provide even fewer services in the future (ADA news, May 12, 1980).
