Summary
The influence of plasma expanders on hemodynamics of the placenta, the maternal blood pressure and kidney function was investigated in 20 patients with EPH gestosis. The plasma expanders were administered under surveillance of the central venous pressure (CVP), measured in the superior vena cava. In 18 of the patients, abnormally low values of the CVP similar to those in patients suffering from severe shock, polytraumatisation or extensive burns were observed. In the remaining 2 patients, the CVP was within the normal range due to effects of early stages of right heart insufficiency. In these cases the administration of volume expanders is contraindicated or should only be attempted with simultaneous surveillance of the CVP. When indicated, diuretics or digitalis should be given. In 5 patients, the hemodynamics of the placenta was examined. Improved placental blood flow was obtained following plasma expander therapy. The use of the complamin test in the diagnosis of pathological placental blood flow has been discussed. As further actions of plasma expanders, decrease in diastolic blood pressure and increased urinary excretion after diuretics have been observed. The patho-physiological mechanisms for these actions and their therapeutic applications have been discussed.
Zusammenfassung
Bei 20 Patientinnen mit EPH-Gestose wurde der Einfluß von Plasmaexpandern auf die Hämodynamik der Placenta, den mütterlichen Blutdruck und die Nierenfunktion untersucht. Plasmaexpander wurden verabreicht unter Kontrolle des zentralen Venendrucks, gemessen in der Vena cava superior (ZVD). Bei 18 Patientinnen fanden sich erheblich erniedrigte Werte des ZVD, wie sonst bei schweren Schockzuständen bei Patientinnen mit Polytrauma oder ausgedehnten Verbrennungen. Bei 2 Patientinnen war der ZVD annähernd im Normbereich infolge gleichzeitig bestehender Rechtsherzinsuffizienz. Es wird darauf hingewiesen, daß Volumenzufuhr in diesen Fällen gefährlich sein kann und nur unter laufender Kontrolle des ZVD vorgenommen werden darf. Gegebenenfalls ist eine Entwässerung und Digitalisierung nötig. Bei 5 Patientinnen wurde die Hämodynamik der Placenta untersucht. Unter Plasmaexpandertherapie konnten verbesserte Durchblutungsverhältnisse nachgewiesen werden. Der Complamintest zum Nachweis pathologischer Placentadurchblutung wird diskutiert.
Als weitere Wirkung von Plasmaexpandergabe wurde ein diastolischer Blutdruckabfall sowie vermehrte Urinausscheidung nach Diuretica gesehen. Die pathophysiologischen Mechanismen dieser Befunde und die daraus resultierenden therapeutischen Konsequenzen werden diskutiert.
