Summary
The capillary changes and the cellular infiltrate were investigated in 13 patients with atopic dermatitis by means of histochemical and quantitative enzyme-cytochemical methods. The changes of the capillaries are comparable to those seen in psoriasis vulgaris; differing from psoriatic lesions the “cotton-ball”-like tortuation of capillaries is less expressed in atopic lesions.
The dermal infiltrate, being preferentially located in the perivascular area, mostly consists of lymphocytes and numerous monocytes. Mast cells show an increase in number. Also, connective tissue cells show a marked reaction resulting in an augmentation in number, size and enzymatic activity. Polymorphonuclear granulocytes may be seen, dependent upon secondary injuries (scratching).
The composition of the cellular infiltrate in skin lesions of atopic dermatitis shows the feature of a chronic inflammation and is interpreted as the result of a non-specific pathogenetic action of immunocomplexes in the skin.
Zusammenfassung
Bei 13 Patienten mit Neurodermitis diffusa wurden zur Beurteilung der Capillaren und des Zellinfiltrates histochemische und quantitativ encymcytochemische Untersuchungen durchgeführt.
Die Veränderungen der Capillaren entsprechen denen bei Psoriasis vulgaris, zeigen jedoch eine geringere Tendenz zu „cotton ball“-artiger Schlängelung.
Das vorwiegend perivasculär lokalisierte dermale Infiltrat besteht überwiegend aus Lymphocyten und zahlreichen Monocyten. Daneben findet sich eine deutliche Vermehrung von Mastzellen. Bindegewebszellen zeigen eine deutliche Mitreaktion (Vermehrung, Vergrößerung, Zunahme der enzymatischen Aktivität). Polymorphkernige Granulocyten können in Abhängigkeit von Sekundärveränderungen (Kratzeffekte) beteiligt sein.
Die Zusammensetzung des dermalen Infiltrates bei der Neurodermitis diffusa entspricht somit dem Bild einer chronischen Entzündung und ist als Ausdruck einer unspezifischen gewebepathogenen Wirkung von Immunkomplexen in der Haut zu deuten.
