Summary
Rabbits received a single dose of131J-albumin intravenously. The pool of the isotope in the kidney was determined in intervals of 60, 90, and 600 sec following the injection to correspond to plasma volumes of 14.6; 14.1 and 18.5 ml respectively. It is assumed that the extra-vascular portion of the pool is represented by the slow increase radioactivity observed 90 sec after injection.
131J-albumin was continuously infused over periods of 10 min into the renal artery of rabbits. After discontinuation of the infusion the output of radioactivity via the renal vein was determined. By applying this method it was found that the albumin-pool of the kidney corresponds approximately to 20 ml of plasma, and that it comprises the extra-vascular albumin too.
This finding leads to the conclusion that there is an intimate exchange of albumin between the intra- and extra-vascular space of the kidney. The larger portion of the extra-vascular albumin leaves the kidney with the venous blood.
To determine the mechanisms involved in flow of plasma proteins from intrato extra-vascular space and vice versa, it was determined whether or not a reduction of tubular water reabsorption would affect the concentration of macromolecules (PVP) in renal lymphatic fluid.
The lymphatic flow and the PVP-concentration in renal lymphatic fluid were measured under conditions of weak osmotic diuresis and tubular reabsorption of ≙ 87% creatinine clearance. After application of mannitol and furosemide the tubular reabsorption rate decreased to 62% of the creatinine clearance. At the same time the lymphatic fluid volume decreased, and the PVP-concentration in the renal lymphatic fluid increased significantly.
On the basis of the data presented above two hypotheses are being advanced regarding the exchange of macromolecules between plasma and interstitium and vice versa.

The macromolecules reach the interstitial space by diffusion along a concentration gradient. They are returned to the blood plasma by solvent drag.

The interstitial space contains compartments which allow the diffusion of macromolecules at different rates. The concentrations of macromolecular substances in the interstitial space, therefore, have been calculated too low in comparison with the blood plasma. In this case the diffusion of macromolecules could occur in both directions without additional mechanisms being involved.

60, 90 und 600 sec nach einmaliger i.v. Verabreichung von131J-Albumin wird das Verteilungsvolumen dieser Substanz in der Kaninchenniere durch Direktaufarbeitung des Organs entsprechend einem Plasmavolumen von 14,6; 14,1 und 18,5 ml ermittelt. Der nach 90 sec sich langsam auffüllende Poolanteil wird extravasal vermutet.
In die A. renalis von Kaninchen wird über 10 min131J-Albumin als Dauerinfusion verabreicht und nach Abbruch der Infusion die Menge des über die Nierenvene abfließenden131J-Albumins gemessen. Mit dieser Ausfuhrmethode läßt sich ein Albuminpool in der Niere ermitteln, der etwa 20 ml Plasma entspricht und von dem angenommen wird, daß er auch das extravasale Albumin der Niere enthält.
Daraus folgt mit hoher Wahrscheinlichkeit, daß das extravasal gelangte Plasma-Albumin in der Niere mit dem intravasalen im engen Austausch steht und zum überwiegenden Teil die Niere wieder mit dem Venenblut verläßt.
Zur Ermittlung der Kräfte, die für die Rückführung des extravasalen Plasma-Eiweißes in die postglomerulären Capillaren verantwortlich sind, wird untersucht, ob die Konzentration von Makromolekülen (PVP) in der Nierenlymphe durch Drosselung der tubulären Reabsorption von Wasser abnimmt.
An der Kaninchenniere in schwacher osmotischer Diurese mit einer tubulären Reabsorption 2259 ca. 87% der GFR (Kreatininclearance) werden der Lymphfluß und die PVP-Konzentration in der Nierenlymphe gemessen. Nach Verabreichung von Mannit und Furosemid sinkt die tubulär reabsorbierte Flüssigkeitsmenge auf 62% der GFR, gleichzeitig steigt die PVP-Konzentration in der Lymphe signifikant an, während das Lymphvolumen absinkt.
Hinsichtlich der Kräfte, die maßgebend für die Wanderung der Makromoleküle aus dem Blut in das Interstitium und von dort in das Blut sind, werden zwei Möglichkeiten diskutiert. Die Makromoleküle könnten durch Diffusion längs eines Konzentrationsgradienten in das Interstitium und durch solvent drag wieder in das Blut gelangen. Andereseits ist es denkbar, daß die im Interstitium existente Konzentration and Makromolekülen nur rechnerisch niedriger als im Plasma ist, da das Interstitium in Räume gegliedert ist, die für Makromoleküle unterschiedlich zugängig sind. Bei dieser Annahme wäre eine Diffusion von Makromolekülen nach beiden Seiten ohne Postulierung zusätzlicher treibender Kräfte möglich.
