Summary
Bullous skin lesions characteristic for pemphigus erythematosus (PE) appeared after a period of 6 years in a woman with the established diagnosis of a discoid lupus erythematosus (LE). The observed transformation of discoid LE-lesions into bullous PE-lesions could be confirmed by means of light microscopy. In the present PE-lesions we found acantholytic blister formation and a thickening of the PAS positive basement membrane. The electron microscopical investigation showed separation of tonofilaments from the desmosomes, clumping of tonofilaments around the nuclei, altered keratohyalin granules, loss of desmosomes and villous transformation of plasma membrane, as described in diseases of the pemphigus group. Moreover, disintegration and necrosis of the dermal-epidermal zone occurred, as described in LE.
These findings suggest that in this patient pemphigus erythematosus is a result from the coexistence of lupus erythematosus and pemphigus. The question if this is the case in all patients with PE cannot be answered. This hypothesis, however, should be evaluated in patients with PE which show positive LE cell phenomenon, light hypersensitivity and scarring bullous lesions.
Zusammenfassung
Bei einer Patientin konnten wir über einen Zeitraum von 6 Jahren den kontinuierlichen Übergang eines histologisch gesicherten chronischen Erythematodes (LE) in einen Pemphigus erythematosus (PE) beobachten, der ebenfalls feingeweblich gesichert werden konnte. Außer der typischenAkantholyse fand sich lichtmikroskopisch eineVerdickung der PAS-positiven Basalmembran. Auch elektronenmikroskopisch wurden neben den typischen feinstrukturellen Zeichen der Akantholyse eine Homogenisierung und degenerative Veränderungen der dermoepidermalen Region nachgewiesen, wie sie beim Erythematodes beschrieben sind.
Diese Befunde werden von uns dahingehend interpretiert, daß die jetzt vorliegende Hautkrankheit, die klinisch und histologisch einem PE entspricht, in pathogenetischer Hinsicht alsKoexistenz zwischen LE und PE gedeutet werden kann.
Aufgrund unserer Beobachtung halten wir es für erwiesen, daß eine solche Koexistenz möglich ist. Inwieweit sie als pathogenetische Grundlage des PE im allgemeinen gelten kann, bleibt dahingestellt. Immerhin muß diese Möglichkeit für diejenigen PE-Fälle offen bleiben, die mit positivem LE-Zellphänomen einhergehen, durch Lichtexposition provoziert werden und mit Narben abheilen.
