Summary

There have been only few reports on the effect of aminoglycosides after local application in the middle ear. Since, at least facultatively, ototoxic substances are of great significance in the local treatment of chronic inflammation of the middle ear, the ototoxicity of a special aminoglycosid (amicacinsulfat) was tested in a comparative study in guinea pigs under bland and inflammatory conditions of the middle ear. For this purpose, sound perception was determined by derivation of compound action potentials over two hours, while the tympanum near the round window was perfused with a solution of amicacin. We found not reversible deafness only under bland conditions. The chronic inflammated middle ear with polyposis protected against diffusion of amicacin in the perilymph space.

Zusammenfassung

Bei Perfusion der Paukenhöhle mit einer Amicacinlösung bei blanden Schleimhautbedingungen tritt durch Diffusion über das runde Fenster eine nicht mehr reversible Schallempfindungsschwerhörigkeit auf. Eine chronisch entzündete, polypös veränderte Paukenhöhlenschleimhaut stellt einen Schutz für das Innenohr dar und scheint die Diffusion in den Perilymphraum erheblich zu behindern.
