Tumoren im sacrococcygealen bereich

Summary

In the world literature is reported about 584 tumors of the sacrococcygeal region, 484 of them are analysed exactly. The symptoms of the various tumors are about the same, pain and nerval irritation were seen in the most cases. Rectal-digital examination mostly leads to diagnosis. Computertomography is of special diagnostic value since some years. Because of the localisation of the tumor therapy often is difficult. Operation is the only way to remove the tumor and should be done so radically as possible. Radiation therapy may induce malignant degeneration. Adjuvant chemotherapy till now is without of any effect. In the paper we report about the very seldom malignant meningeoma of the sacrum, in the literature we could not find any other case. Besides this and the 584 cases of the literature there is reported about 4 other own cases of sacrococcygeal tumors, one neurofibroma and three giant cell tumors of the Os sacrum.

Zusammenfassung

Anhand der Weltliteratur werden 484 Fälle von insgesamt 584 beschriebenen Fällen von Sacrococcygealtumoren analysiert. Diese Tumoren verschiedenen Ursprungs zeigen Symptome, die sich nicht wesentlich voneinander unterscheiden (Schmerzen, sensible und motorische Störungen, Miktions-und Defäkationsbeschwerden). Die Diagnose wird am häufigsten durch rektale Untersuchung gestellt, in letzter Zeit bietet die Computertomographie eine wichtige diagnostische Hilfe. Die Tumoren dieser Region bringen wegen ihrer Lokalisation große Behandlungsschwierigkeiten mit sich. Der chirurgische Eingriff ist das Mittel der Wahl und sollte so radikal wie möglich durchgeführt werden. Eine Bestrahlungs-therapie darf mit Rücksicht auf die Tendenz zur malignen Entartung verschiedener Tumoren nur mit äußerster Vorsicht angewandt werden. Über die erfolgreiche Anwendung von adjuvanter zytostatischer Therapie sind noch keine ausreichenden Erfahrungen gesammelt werden.

Zu den 584 Sacrococcygealtumoren aus der Welt-literatur kommen vier eigene Fälle, drei Riesenzelltumoren und ein Neurofibrom. Das bei uns beobachtete — in obiger Zusammenstellung enthaltene —maligne Meningeom am Sacrum stellt eine Rarität dar, in der Weltliteratur konnte kein ähnlicher Fall gefunden werden.
