Summary
Amongst 918 newborn infants who received a total of 1784 exchange transfusions, eleven cases of septicaemia occurred, three ending fatally. Premature infants who received an exchange transfusion showed a lower rate of septicaemia compared with the overall rate of cases of septicaemia given in statistics. In newborn infants with erythroblastosis the incidence of septicaemia seems to be slightly higher than it would be expected from the overall rate in newborns; however, the number of cases we investigated is too small to give conclusive results. On the whole the risk of septicaemia after exchange transfusion is low. Until 1964 exchange transfusions were always coupled with an antibiotic treatment; from 1965 onwards we used antibiotics only in selected cases. Apart from cases of suspected septicaemia we administered antibiotics in cases where the catheterisation of the umbilical vein proved particularly difficult, and in cases where multiple — three or more — exchange transfusions were necessary.
Zusammenfassung
Unter 918 Neugeborenen, die in den Jahren 1958–1967 1784 Austauschtransfusionen erhielten, entstanden elf Septikämien mit drei Todesfällen (1,2%). Bei Frühgeborenen lag die Septikämierate nach Austauschtransfusion eher niedriger als andernorts die Septikämierate für das gesamte Kollektiv der Frühgeborenen. Bei den wegen Sensibilisierung behandelten Neugeborenen waren von der Septikämie nur reifgeborene Kinder betroffen. Hier dürfte die Septikämierate etwas höher liegen als die Durchschnittsrate für alle Neugeborenen, jedoch ist unser Kollektiv nicht groß genug für eine sichere Aussage. Das Risiko einer Septikämie als Folge einer Austauschtransfusion ist also gering. Bis zum Jahre 1964 wurde die Austauschtransfusion stets mit einer antibiotischen Behandlung kombiniert, ab 1965 gaben wir Antibiotica nur auf besondere Indikation. Hierzu gehörte neben Verdacht auf Septikämie auch eine erschwerte Katheterisierung der Nabelvene und mehrfache Wiederholung der Austauschtransfusion.
