Abstract
In a liveborn infant with 69 chromosomes some symptoms have been found which were also seen among the cases of triploidy reported before. The characteristic face, syndactyly, coloboma, abnormal neurological and sexual development, and increased number of digital whorls are the most frequent findings. In our case we found arhinencephaly, bilateral microphthalmy with colobomata iridis, cheilognathopalatoschisis, syndactyly of the III–IV finger, bilateral simian crease, mola hydatiformis of placenta.
There was strong Rh immunization of the mother who was rh negative, possibly due to an increased erythrocyte antigen strength, because of triple gene dosage of the child.
Zusammenfassung
Ein frühgeborenes weibliches Kind mit ausgeprägten Mißbildungen wird beschrieben. Hypertelorismus, Mikrophthalmie mit beiderseitigem medialem Kolobom und mittlerer Gaumenspalte waren die wichtigsten Symptome. Die Ohrmuscheln standen tief und waren schlecht modelliert. Rechts bestand Syndaktylie des 3. und 4. Fingers, links nur zum Teil. Die Zehen waren verschieden lang, die Nägel hypoplastisch. Das Gehirn war pathologisch-anatomisch sehr verändert ohne Hemisphären und große Lobi, mit Aplasie der Falx cerebri und des Tractus opticus. Histologisch bestand das Gehirn aus unreifen Gliazellen mit hyperchromatischen Kernen und stark reduzierten Ganglienzellen. Das Herz hatte einen ausgeprägten Defekt des Septum ventriculi, die rechte Lungenarterie war stenotisch. Die linke Niere war lobulär mit cystischer Veränderung der Nebennierenrinde. Die Alveolen der Lunge waren von Amnionflüssigkeit und Leukocyten erfüllt, die Leber wies herdförmige Nekrosen der Leberzellen und Leukocyten auf.
Das Kind zeigte schwache Lebenszeichen und starb 12 Std nach der Geburt. —Die cytogenetische Analyse des peripheren Blutes zeigte einen Karyotyp mit 69 Chromosomen, dabei XXX-Gonosomen. Ein einfacher Drumstick war in 5% der Granulocyten nachweisbar. Die wichtigsten Mißbildungen der bisher veröffentlichten Triploidiefälle sind tabellarisch zusammengestellt und werden diskutiert. Eine starke Rh-Immunisierung der Mutter könnte mit der dreifachen Gendosis beim Kind zusammenhängen.
