Zusammenfassung
Autoradiographische Untersuchungen bei frischen Schenkelhalsfrakturen, Schenkelhalspseudarthrosen mit oder ohne gleichzeitige Kopfnekrose und Schenkelkopfnekrosen ohne Pseudarthrosen lassen folgende Rückschlüsse ziehen:
Nach einer medialen Schenkelhalsfraktur ist besonders das gewichttragende, obere Drittel des Schenkelkopfes von der Zirkulation ausgeschlossen, während um die Fovea meist normale Durchblutungsverhältnisse vorliegen.
Der Frakturspalt kann trotz einer Schenkelkopfnekrose ossifizieren. Das Ergebnis der Nagelung in bezug auf Reposition und erzielte Stabilität erscheint für die Heilung der Fraktur wichtiger als die Durchblutungsverhältnisse im Schenkelkopf.
Die Revitalisation und der Knochenneuaufbau des Schenkelkopfes erfolgt nicht von der meist erhaltenen Arterie des Ligamentum teres, sondern vom Schenkelstumpf aus. Der gewichttragende Femurkopfanteil bleibt am längsten außerhalb der Zirkulation.
Eine Kopfnekrose muß nicht immer von Einbrüchen gefolgt sein. Die nekrotischen Zonen können sich unter Schrumpfung des Schenkelkopfes langsam resorbieren und der neu aufgebaute Schenkelkopf ist danach erheblich kleiner als normal.
