Sekundäre carcinome in primär gutartigen mischtumoren der speicheldrüsen

Secondary carcinoma in primary non-malignant mixed tumors of the salivary glands

Summary

A mixed tumor of a salivary gland behaving malignantly from the beginning is certainly rare if exists at all. But there are mixed tumors behaving benignly for 15 years or more that begin suddenly to rapidly metastasize and kill the patient within a short time. The histological examination shows that in the old benign mixed tumor a totally new type of tumor has arisen, a cancer which soon destroys the mixed tumor. Morphologically most of the new cancers in our series showed a combination of squamous cell carcinoma and adenocarcinoma, a socalled adenoakanthoma. Secondary cancers in primary benign mixed tumors are rare (2–3%) of all cases of mixed tumor). These highly malignant tumors could perhaps be avoided if all mixed tumors were operated on immediately and not given time to age.

Zusammenfassung

Ein Speicheldrüsenmischtumor, der sich von Anfang an bösartig verhält, ist — wenn es ihn iiberhaupt gibt — sicher außerordentlich selten. Es kommen dagegen, wie eigene Beobachtungen zeigen, Mischtumoren vor, die sich 15, 20 und mehr Jahre gutartig verhalten, dann aber plötzlich rasch wachsen und metastasieren und daher meist schnell zum Tode des Patienten führen. Histologische Untersuchungen zeigen, daß im alten gutartigen Mischtumor ein gänzlich neuer Tumortyp, ein Carcinom entstanden ist, das seinen Mutterboden zerstört und metastasiert. Morphologisch entsprechen these sekundären Carcinome häufig Mischformen von Plattenepithel- und Adenocarcinomen, sogenannten Adenoakanthomen. Auch die Entstchung von Carcinomen in Mischtumoren ist selten (2–3% aller Fälle von Mischtumoren), dieses Ereignis wird sicher noch seltener, wenn alle Speicheldrüsenmischtumoren sofort operiert werden und ihnen nicht Zeit gelassen wird alt zu werden.
