Zur Pathogenese der Patellaluxation

Zusammenfassung

Aus der klinischen Beobachtung läßt sich ber genauer Analyse der habituellen und rezidivierenden Patellaluxation ein unterschiedlicher funktionsmechanischer Ablauf ablesen. An Hand funktioneller Röntgenstudien und Versuchen mit einem vereinfachten Skeletmodell des Beines beweisen diese eine unterschiedliche Pathomechanik der habituellen und der rezidivierenden Patellaluxation. Die pathologische Ausgangslage der Patella (Patella alta et lateralisata) ist die entscheidende Voraussetzung für beide Luxationen. Die mitwirkende Bedeutung anderer pathologischer Veränderungen findet ihre weitere Klärung. Eine Fehlinnervation des M. quadriceps bei rezidivierender Patellaluxation konnte auch mittels des EMG nicht nachgewiesen werden. Die Ursache der Patella alta et lateralisata bleibt weiterhin offen.

Summary

The clinical study of habitual and recurrend luxation of the patella shows a different way of function. With functional X-ray-studies and also experiments with a simple model of leg and knee, it could be shown a the different pathological mechanism of the two forms of patellaluxation. The precondition for these luxations is the patella alta and lateralisata. The signifikation and the effect of other pathological signs were analysed. A disinnervation of the m. quadrizeps as reason of the recurrend luxation could even with EMG not be found. The cause of the patella alta and lateralisata still is unknown.

Résumé

L'analyse clinique de la luxation habituelle de la rotule ou bien récidivante nous montre des fonctions differentes au point de vue mécanique. Des radiogrammes fonctionelles et des éxperiments d'un model de la jambe et du genou prouvent la différence des mécanismes pathologiques. La rotule alta et lateralisata est la seul condition des luxations. La signification et l'effet d'autres signes pathologique furent analysés. Les luxations récidivante n'avaient pas d'innervation incorrecte du m. quadrizeps, mème pas sous controle de l'EMG. La cause da la rotule alta et lateralisata reste obscure.
