Summary
For studying end-diastolic distensibility of the left ventricle following acute changes of the systemic pressure experiments in 24 dogs with untouched thorax in morphine-urethane anesthesia were performed. The enddiastolic volume was determined by the dye-dilution method, and correlated to the enddiastolic left ventricular pressure recorded simultaneously.
This pressure-volume relation representing the diastolic distensibility of the intact heart was found to be almost constant under normal conditions as well as under the conditions of arterio-venous shunt, temporary occlusions of the central aorta, and acute pressure rise by double carotid occlusion; under elevation of the systemic pressure a tendency of the distensibility curve toward its right was observed more often than not. In four experiments the dislocations of the distensibility curves were significant or probable (maximum 14,1%), but were not clearly related to the direction and the degree of the systemic pressure changes. The possible causes for a variability of the diastolic pressure-volume relations happening to the heart in situ are discussed.
The presented findings do not suggest that changes of the enddiastolic ventricular distensibility can be considered as a significant factor contributing to the adaptation of the heart to acute pressure loadings.
Zusammenfassung
An 24 Hunden mit geschlossenem Thorax in Morphin-Urethannarkose wurden die enddiastolischen Druck-Volumenbeziehungen des linken Ventrikels bei akuten, mechanisch und reflektorisch ausgelösten Änderungen des arteriellen Drucks untersucht. Die Korrelation der nach der Farbstoffverdünnungsmethode bestimmten enddiastolischen Volumina mit den synchron registrierten enddiastolischen Ventrikeldrucken ergab unter den Bedingungen des arteriovenösen Kurzschlusses, der Aortenokklusion und des akuten Entzügelungshochdruckes bei Vergleich mit den Kontrollwerten im Mittel eine weitgehende Konstanz der diastolischen Dehnungskurve, mit geringer Tendenz zur Rechtsverlagerung, d. h. relativer Volumenzunahme bei erheblicher Steigerung des Systemdrucks. In Einzelfällen waren signifikante Verlagerungen der Dehnungskurve nach beiden Seiten um maximal 14,1% zu verzeichnen, die jedoch in keiner eindeutigen Beziehung zu Richtung und Ausmaß der Systemdruckänderungen standen. Die möglichen Ursachen einer Variabilität der diastolischen Druck-Volumenrelation beim Herzen in situ werden erörtert.
Die vorliegenden Befunde sprechen nicht dafür, daß Änderungen der enddiastolischen Druck-Volumenbeziehungen als Grundlage einer Anpassung des Herzens an akute Druckbelastungen zu betrachten sind.
