Zusammenfassung
Es wird untersucht, ob und wieweit die Lage des Diffusionswiderstandes bei der alveolär-capillären O2-Diffusion von Bedeutung ist. Dazu wird der Vorgang der O2-Druckangleichung vergleichend für zwei extreme Modellfälle analysiert: 1. der ganze O2-Diffusionswiderstand liegt in der „Alveolarmembran“, 2. der gesamte O2-Diffusionswiderstand liegt im Innern der Erythrocyten. Insbesondere wird untersucht, von welchen Faktoren in den beiden Modellen die alveolär-endcapilläre O2-Druckdifferenz sowie der O2-Diffusionsfaktor (= O2-Diffusionskapazität) abhängen. Die Grenzen der Anwendungsmöglichkeit der Modelle auf den alveolären O2-Austausch in der Lunge werden diskutiert.
Summary
The question if and to what extent the site of the resistance to diffusion is of importance for the alveolar-capillary O2 diffusion is examined in theory. For this purpose the process of O2 tension equilibration is analyzed for two extreme model cases: 1. the total resistance to O2 diffusion lies in the “alveolar membrane” (“membrane resistance model”), 2. the total resistance to O2 diffusion is located in the interior of the erythrocytes (“erythrocyte resistance model”). The factors determining the alveolar-endcapillary O2 pressure difference and the O2 diffusing capacity are examined. The more important results of the analysis are:

The approaches to a quantitative analysis of the O2 exchange are basically different for the two model cases.

The influence of changes in the total resistance to O2 diffusion, in the slope of the O2 dissociation curve of the blood, in the perfusion rate, in the capillary volume and in the contact time upon the O2 tension equilibration is theoretically different for the two models, but the differences are so slight that they can be neglected in any practical application.

However, if the concentration of erythrocytes (hematocrit) is changed, major differences in the mode of O2 tension equilibration between the two models occur. For the “erythrocyte resistance model” the O2 diffusing capacity is proportional to the erythrocyte concentration, for the “membrane resistance model”, independent of it. This difference could be utilized to determine if in the O2 exchange in the lung the major part of the resistance to O2 diffusion is located in the “alveolar membrane” or in the erythrocytes.
The limitations of the application of the models to the analysis of alveolar O2 exchange in the lung are discussed.
