Summary
Clinical and electroencephalographical findings of 115 patients (72 male, 43 female) with epilepsy of late onset (first manifestation after age of 45) are reported and the course of the disease (average time of observation 6–7 years) is observed by follow-up studies.
In 22 cases the pathogenesis remained uncertain, in 50 cases cerebral arteriosclerosis or heart and circular disturbances were probable causes.
In 9 cases brain tumors were found later, although cases with primarily diagnosed brain tumors were previously excluded form the series.
According to clinical and EEG criteria, temporal-lobe epilepsy is prevalent. EEG changes, especially with typical seizure activity were rare. In 43.7% the EEG's were normal.
More than one third of the patients without brain tumors became completely free of attacks during the follow-up period. In more than one third, the attacks had become rare or slighter, only one fourth of the patients had the same or an increased number of attacks.
The results correspond with clinical experiences on the relatively benign evolution of epilepsy of late onset.

Es werden die klinischen und elektrencephalographischen Befunde von 115 Patienten (72 männlich, 43 weiblich) mit sogenannter Spätepilepsie (Erstmanifestation nach dem 45. Lebensjahr) mitgeteilt und der Krankheitsverlauf (Beobachtungszeit durchschnittlich 6–7 Jahre) katamnestisch kontrolliert. Ursächlich blieben 22 Fälle ungeklärt, in 50 Fällen waren eine cerebrale Arteriosklerose oder Herz- und Kreislaufstörungen die wahrscheinlichste Ursache. Hirntumoren fanden sich noch in 9 Fällen im weiteren Verlauf, obgleich zunächst Fälle mit primär nachweisbarem Hirntumor ausgeschlossen waren. Nach klinischen und EEG-Kriterien überwiegt die Temporallappen-Epilepsie. EEG-Veränderungen, insbesondere spezifische Krampfpotentiale waren selten, in 43,7% war das EEG normal. - Über 1/3 der Patienten ohne Hirntumor wurden im weiteren Verlauf völlig anfallsfrei. In über 1/3 waren die Anfälle seltener oder leichter geworden, nur 1/4 der Patienten hatte im weiteren Verlauf unvermindert Anfälle oder eine Zunahme der Anfallshäufigkeit. Die Ergebnisse bestätigen allgemeine klinische Eindrücke über die relativ gutartigen Verläufe von Spätepilepsien.
