Summary
Flow reversal could be shown, by means of a directional ultrasonic flow detector designed to record pulse curves of the ophthalmic artery, in 31 out of 37 cases of complete occlusion of the internal carotid artery. Five showed no Doppler pulsation in the region of the medial canthus and only one case showed physiological streaming due to an abnormal origin of the ophthalmic artery. —Flow reversal was also demonstrated in 9 out of 20 cases of highgrade stenosis of the internal carotid artery, and in both of two cases of total occlusion of the common carotid artery. Reversed flow could be stopped (and in some cases altered to the normal direction) by digital compression of the external maxillary and anterior temporal arteries.
Résumé
L'auter a étudié la revascularisation à l'aide d'un débimètre ultrasonique directionnel destiné à enregistrer les pulsations de l'artère ophtalmique dans 31 sur 37 cas d'occlusion de l'artère carotide interne. Dans cinq cas, on ne relevait aucune pulsation Doppler dans la région du canthus médian. Dans un seul cas, on notait un flux physiologique dû à une origine anormale de l'artère ophtalmique. L'inversion du flux fut également démontrée dans 9 cas sur 20 de sténose très importante de l'artère carotide interne et dans deux cas d'occlusion totale de l'artère carotide primitive. La revascularisation put être arrêtée (et modifiée dans certains cas pour obtenir une direction normale) par la compression digitale des artères maxillaires externe et temporale antérieure.
Zusammenfassung
Mit der Doppler-Ultraschall-Diagnostik konnte die Pulskurve der Â. ophthalmica registriert. werden. Dabei fand sich in 31 Fällen von 37 Patienten, die einen kompletten Verschluß der A. carotis interna aufwiesen, eine Strömungsumkehr. Auch bei hochgradigen Stenosen im Bereich der A. carotis int. ließ sich diese Strömungsumkehr nachweisen. Durch digitale Kompression der Maxillar- und Temporal-Arterien wurde die Strömungsumkehr aufgehoben.
