Accidental intrathecal vincristine administration

Summary

A case is described of accidental intrathecal administration of vincristine, with detailed clinical observations over a 17-day period. The clinical picture resembled that seen with toxicity from intravenously administered vincristine, but was rapidly progressive and resulted in death. The onset was characterised by opisthotonos, followed by ascending paralysis and finally bulbar and cerebral involvement. The specific changes in the brain at autopsy were masked by those due to prolonged artificial respiration prior to death.

The accidental drug administration occurred owing to procedural errors, which can be avoided by strict attention to rules.

Zusammenfassung

Es wird ein Fall von ungewollter intrathekaler Vincristine-Applikation beschrieben, der klinisch genau während 17 Tagen verfolgt werden konnte. Das klinische Bild ähnelt jenem, wie man es nach intravenöser Vincristine-Applikation sieht, war jedoch rascher progredient und führte zum Tode. Zu Beginn fand sich ein Opisthotonus, dann eine aufsteigende Lähmung, die schließlich von bulbären und cerebralen Symptomen gefolgt wurde. Bei der Autopsie waren die spezifischen Veränderungen verdeckt durch jene, die auf eine langdauernde künstliche Beatmung vor dem Eintritt des Todes zurückzuführen waren. Die ungewollte intrathekale Verabreichung war auf ein fehlerhaftes Vorgehen zurückzuführen, welches bei strenger Beachtung der Applikationsregeln vermieden werden könnte.
