Summary
Double ring formation in single radial immunodiffusion for light chains of type kappa and not for light chains of type lambda was observed in cerebrospinal fluid in 24 out of 42 patients with multiple sclerosis, 1 patient with cerebral cysticercosis, 1 of 3 patients with neurosyphilis and 1 patient with spastic paraparesis of unknown eitology. Double ring formation was not observed in cerebrospinal fluid of 73 patients with other various diseases of the nervous system. All patients with double ring formation showed deep Mastix curves of the left type. All of them except one with multiple sclerosis showed increased immunoglobulin G values and increased immunoglobulin G ratios. The test is simple and requires only 6 μl of unconcentrated cerebrospinal fluid. In the analysis of cerebrospinal fluid immunoglobulins it could be applied as a further diagnostic aid in establishing a diagnosis of multiple sclerosis.
Zusammenfassung
In der radialen Immunodiffusion des Liquors wurden Doppelringe bei Bestimmung der leichten Ketten vom Typ Kappa, nicht jedoch vom Typ Lambda beobachtet. Die Doppelringe traten auf bei 24 von 42 Patienten mit multipler Sklerose, 1 Patienten mit cerebraler Cysticercose, 1 von 3 Patienten mit Neurosyphilis und 1 Patienten mit ungeklärter Paraspastik der Beine. Im Liquor von 73 Patienten mit anderen Erkrankungen des Nervensystems wurde der Doppelring nicht gesehen. Alle Liquors mit Doppelring zeigten einen tiefen Linksausfall der Mastix-Kurve. Mit Ausnahme eines Liquors bei multipler Sklerose hatten alle erhöhte Immunoglobulin G-Konzentrationen und erhöhte Immunoglobulin G-Quotienten.
Die Untersuchung ist leicht durchführbar, man benötigt nur 6 μl uneinengten Liquors. Die Untersuchung könnte die Diagnose „multiple Sklerose“ sichern helfen.
