Primary treatment of comminuted intertrochanteric fractures of the hip with a Leinbach prosthesis

Summary

The authors present 43 cases of comminuted intertrochanteric fracture, treated by replacement with a Leinbach prosthesis. The average age of the patient was 81 years, the average time of operation was 70.8 min, and the average blood loss was 502 cc. 88% of the patients were walking within 4–10 days. One deep infection occurred requiring removal of the prosthesis five months later. Based on this experience, the authors recommend consideration of the use of a Leinbach prosthesis in the primary treatment of the elderly patient with a comminuted intertrochanteric fracture of the hip.

Résumé

Les auteurs présentent 43 cas de fracture inter-trochantérienne, comminutive, traitées par mise en place d'une prothèse de Leinbach. L'âge moyen des blessés était de 81 ans, la durée moyenne de l'opération a été de 70,8 mn et la perte sanguine moyenne de 502 cc. 88% des opérés ont pu reprendre la marche entre le 4è et le 10è jour. Il n'y a eu à déplorer qu'une seule infection profonde, qui a obligé à pratiquer l'ablation de la prothèse au 5è mois post-opératoire. En se basant sur leur expérience, les auteurs estiment que la prothèse de Leinbach peut avoir des indications dans le traitement des fractures inter-trochantériennes comminutives du sujet âgé.
