Summary
The clinical features and electromyographic findings in two patients with poisoning subsequent to handling an aqueous dispersion of vinylidene chloride are described. The neurological symptoms correspond to the “polyneuritis cranialis” as commonly seen in poisoning due to the use of trichloroethylene.
Within 8 to 30 hours of the exposure to the toxic agent both patients developed sensory disturbances in the trigeminal area of the face, mouth and tongue, spreading to the 2nd, or 2nd and 3rd cervical segments later on. Only one patient showed motor weakness of the jaw muscles, the lateral recti muscles of the eyes and of the tongue muscles.
The electromyographic evaluation of the trigemino-facial reflexes revealed the functional disorder to involve mainly the interneuronal system. In discussing the possible etiological factors of the intoxication, evidence suggests that ethylene dichloride or vinylidine chloride (1,1-dichloroethylene) are toxic agents.
Zusammenfassung
Es wird über zwei Patienten berichtet, bei denen sich nach Kontakt mit einer wäßrigen Kunststoffdispersion von Vinylidenchlorid Vergiftungserscheinungen entwickelten, wie sie im Umgang mit Trichloräthylen bekannt geworden sind. Bei beiden Kranken stellten sich nach einer Latenz von 8 bzw. 30 Std, an den Lippen und an der Mundschleimhaut beginnend und sich von hier rasch über das Gesicht und über die Segmentversorgungsbereiche C 2 bzw. C 2 und C 3 ausbreitend, eine dissoziierte Sensibilitätsstörung mit Analgesie, Thermanaesthesie und starker Hypaesthesie ein. Während in dem ersten Fall weitere Nervenstörungen ausblieben, traten in dem zweiten passagär auch noch motorische Störungen von Seiten des N. trigeminus, beider Nn. abducentes und des N. hypoglossus auf. Letztgenannte Nervenstörungen schwanden nach 2–6 Wochen wieder vollkommen, die sensible Trigeminusschädigung erwies sich jedoch in beiden Fällen als irreparabel. Bemerkenswert erscheint dabei das Ergebnis der myographischen Untersuchung der Gesichtsreflexe, weil dieses nicht allein im Sinne einer Schädigung peripherer Trigeminusneurone, sondern vor allem auch einer Funktionsstörung in der Interneuronenkette des Hirnstammes gedeutet werden muß. Unseres Erachtens ist auf Grund der bisherigen Kenntnisse anzunehmen, daß der Intoxikationsschaden durch Mono- oder Dichloracetylen verursacht wurde. Die Beobachtungen mahnen zur Vorsicht beim Umgang mit Zwischenprodukten von Polyvinylidenchlorid, zumindest sofern besondere physikalische Umstände wie beispielsweise starke Hitzeeinwirkungen, vorliegen.
