Summary
A microscopical examination of 3 cases showing delayed posttraumatic rupture of the aorta 3 and 4 days after thoracic contusion caused by car accidents. The aortic wall near the rupture shows an ischaemic necrosis involving all layers (intima and adventitia), blending continuously into the undamaged wall regions. Slight infiltration of leucocytes into the rupture cleft and the neighbouring tissue of the vessel. Patchy and focal increase of acid mucopolysaccharides. Degeneration of the elastic fibres. The microscopical findings lead to the conclusion of a delayed (two-staged) rupture. The final rupture of the remaining layers is mainly thought to be due to posttraumatic necrosis. A considerable degree of atherosclerosis partly favours the onset of the rupture, a fact that should be considered when giving an opinion in civil litigation. Zusammenfassung
Histologische Untersuchung von 3 Fällen mit zweizeitiger posttraumatischer Aortenruptur 3 bzw. 4 Tage nach Verkehrsunfall mit stumpfem Brustkorbtrauma. Beiderseits des Aortenrupturspalts findet sich eine ischämische Gefäßwandnekrose mit Beteiligung der Intima und Adventitia, welche kontinuierlich in die unveränderte Gefäßwand übergeht. Geringe Entzündungszellinfiltrate in den Rupturspalten und deren unmittelbarer Umgebung. Fleckförmige und herdförmige Vermehrung der sauren Mucopolysaccharide. Degenerative Veränderungen der elastischen Fasern. Aus den Befunden ist eine Zweizeitigkeit ablesbar. Bei zweizeitigen Rupturen mit erheblicher Atherosklerose ist eine Mitursächlichkeit der Gefäßerkrankung bei privatversicherungsrechtlicher Begutachtung nachweisbar.
