The effect of immunological and non-immunological factors on corneal graft survival: A single centre study

Summary and general discussion

The objective of this thesis was to study factors influencing corneal graft survival. These factors can be classified as non-immunological or immunological.

The main non-immunological factor which has increased graft survival since 1939 is the improvement in surgical techniques following the introduction of the operation microscope. A linear relation between the improved surgical skill and the number of less favourable cases which could be operated was observed in many corneal transplantation centers. Patients with highly vascularized corneas and previous corneal graft failures were no longer excluded from corneal transplantation. At the present time, corneal graft rejection due to immunological factors is the ultimate determinant of corneal graft survival.

Of the immunological factors which may influence corneal graft survival, we have studied the effect of the major histocompatibility complex (MHC) and theABO blood group system. The human MHC (HLA) is a genetic system that contains information for polymorphic cell membrane determinants. HLA antigenic incompatibilities provoke strong immunological reactions in tissue transplantation. Furthermore, genes within the HLA-system influence the individual degree of susceptibility to diseases implicating the immune system.
