Abstract
Diverticulosis of the colon is common in the descending and sigmoid part; the right colon is rarely involved. Inflammation or perforation of a cecal diverticulum is an uncommon condition that mimics acute appendicitis. The correct diagnosis is mostly made after operative exploration. Four cases were surgically treated between 1994 and 1995. Because of the severe inflammation, ileocecal resection or right hemicolectomy was necessary; in one case we performed a local excision. After removal of the inflamed mass the postoperative healing was uncomplicated. A malignancy could be histologically excluded in all cases. As in sigmoid diverticulitis, early resection seems to be a safe operative therapy. Laparoscopy can show the correct diagnosis before the laparotomy is carried out. Preoperative diagnostic measures, differential diagnosis and possible surgical procedures are discussed.
Zusammenfassung
Die Hauptlokalisation der Kolondivertikulose liegt im Bereich des Colon descendens und Sigma, sehr selten im Bereich des Zäkums. Die Entzün-dung oder Perforation eines Zäkumdivertikels wird aufgrund ihrer Seltenheit and schwierigen Abgrenzbarkeit zur akuten Appendizitis oft erst intraoperativ diagnostiziert. Wir berichten fiber unsere Erfahrungen mit 4 Patienten, diezwischen 1994 and 1995 operativ behandelt wurden. Wegen ausgeprägter entzündlicher Veränderungen erfolgte die Ileozäkalresektion bzw. Hemikolektomie rechts, 1mal genügte eine Exzision and Übernähung. Nach Entfernung des befallenen Bereichs kam es zu einem unkomplizierten postoperativen Verlauf, ein Karzinom konnte in allen Fäl-len histologisch ausgeschlossen werden. Wie bei der komplizierten Sigmadivertikulitis stellt die frühe Resektion eine erfolgreiche chirurgische Therapie der. Die Laparoskopie kann dabei die endgültige Diagnose schon vor der Laparotomie zeigen. Preoperative Diagnostik, Differentialdiagnosen und chirurgische Therapiemöglichkeiten werden diskutiert.
