Summary
The first part of this study concerns the still little known clinical picture of choroideremia, while the second part deals with the rare affection known as gyrate atrophy of the choroid and retina.
Both parts first present a review of the changes described by other authors and a discussion of the mode of inheritance of these hereditary eye diseases, followed by a report on a personal investigation of cases found in the Netherlands and a comparison of our findings with those of other authors.
Choroideremia
The nine pedigrees (including a large one combining four others) included 52 patients and 96 carriers; 45 patients and 61 carriers were examined by us.
Patients
The first fundus changes seen in six patients, the youngest of whom was 1 1/2 years old, were fine granular pigment accumulations at the equator and periphery. They were localized deep in the retina at the level of the pigment epithelium. Not a trace of choroidal atrophy was seen in this initial stage. Subsequent stages showed fundus changes which are readily confused with those of other fundus affections such as gyrate atrophy of the choroid and retina on the one hand, and atypical tapetoretinal degeneration with choroidal sclerosis on the other.
The 45 patients included 28 myopes, 12 emmetropes and four hypermetropes; in one, refraction was not determined. Emmetropia was mainly seen in the incipient cases. In 11 of the 13 patients in whom refraction was followed up, progression of the myopia was seen during the period of observation.
Dark adaptation, visual fields, ERG and EOG were found normal in the initial stage. Unmistakable pathological changes occurred in these values in subsequent stages.
Carriers
The carriers showed widely varied fundus pictures, ranging from mildly abnormal pigmentation to features closely resembling the changes which characterize the choroideremia patient.
Three carriers from three different pedigrees showed both the subjective and objective features normally found in patients. In addition to night blindness, these women showed fundus changes resembling those seen in patients in an advanced stage, and pathological changes in dark adaptation, visual fields, ERG and EOG. On the other hand, one carrier (daughter of a patient) showed no fundus changes at all.
An exceptional fundus picture with the aspects of a tapetum-like reflex as described by Mann, was observed in one woman. It is not known whether she is a carrier or not.
In three pedigrees the choroideremia occurred in association with another X-chromosomal eye affection (protanopia in two pedigrees, and deuteranopia in one). In all three pedigrees, crossing-over could be assumed as highly probable.
To explain, on the one hand the complete clinical picture of a patient seen in three carriers, and on the other hand the complete absence of changes in one carrier, alterations in the expressivity of the pathological gene or possibly of the allele are suggested.
In view of the fact that no trace of choroidal atrophy was seen in the initial stage, it seems probable that the primary seat of the affection lies in the pigment epithelium (tapetum nigrum). Consequently we accept Waardenburg's suggestion to change the designation choroideremia into progressive tapetochoroidal degeneration.
Gyrate atrophy of the choroid and retina
The five pedigrees included 13 patients, 11 of whom were examined. The chief complaints were found to result from myopia and night blindness occurring early in life.
Objectively, the initial stage of the affection was characterized by the presence of sharply defined, garland-shaped, peripherally localized areas of choroidal and retinal atrophy; in the later stages these spread in the direction of the posterior pole, and often connect with an annular peripapillary atrophic field. Myopia was found in 11 cases; it showed progression in a relatively large percentage of these cases.
The choroidal and retinal atrophy showed a more central localization in one hypermetropic patient.
Our findings also confirmed the high rate of complicated cataract in gyrate atrophy; this type of cataract was found in six of our 13 patients.
The autosomal recessive inheritance of gyrate atrophy was confirmed by our findings. The consanguinity found in three of the five families is another argument in favour of this.
Association of gyrate atrophy with complicated heterochromia (Fuchs) was observed in one female patient.
Alder's anomaly was not demonstrable in the six patients examined for this purpose.
Special attention has been given to the concomitance of tapetoretinal degeneration and gyrate atrophy of the choroid and retina, which a few authors have described, but which we were unable to confirm.
Résumé
La première partie de cette étude se rapporte au syndrome encore peu connu de la choroïdérémie, la seconde partie a trait à l'affection peu fréquente dénommée atrophie gyrée de la choroïde et de la rétine.
Après s'être attaché dans chacune des deux parties aux troubles décrits par d'autres auteurs et au mode de transmission de ces maladies héréditaires de l'oeil, l'auteur fait le point d'une enquête concernant un certain nombre de cas qui se sont présentés aux Pays-Bas et termine par une comparaison entre ses propres constatations et celles d'autres auteurs.
Choroïdérémie
Dans les neuf arbres généalogiques, dont l'un, très ramifié, résultant de la jonction de quatre autres, l'on put relever la présence de 52 hommes atteints et de 96 femmes conductrices, dont respectivement, 45 et 61 se sont soumis à notre examen.
Hommes atteints
Comme premières altérations du fond de l'oeil, on trouva chez 6 hommes atteints, dont le plus jeune avait un an et demi, de fines granulations pigmentées dans l'équateur et dans la périphérie. Elles étaient situées profondément dans la rétine, à la hauteur de l'épithélium pigmenté. Dans cette phase initiale aucune trace d'atrophie choroïdienne ne fut constatée. Dans les phases suivantes l'on observa des altérations du fond de l'oeil faciles à confondre avec d'autres affections du fond de l'oeil, telles que l'atrophie gyrée de la choroïde et de la rétine, d'une part, et, de l'autre, la dégénérescence tapéto-rétinienne atypique avec la sclérose choroïdienne.
Sur les 45 hommes atteints, 28 étaient myopes, 12 emmétropes, 4 hypermétropes et 1 n'a pas été vérifié. L'emmétropie fut constatée dans les cas débutants. Dans 11 des 13 cas où il fut possible de suivre la réfraction, une progression de la myopie fut constatée au cours de la durée d'observation. Dans les phases initiales, la courbe d'adaptation à l'obscurité, les champs visuels, l'électro-rétinographie et l'électrooculographie etaient normaux. Au cours des phases ultérieures on y vit apparaître de très nettes altérations pathologiques.
Femmes conductrices
Les conductrices présentèrent des aspects très variés du fond de l'oeil. Ces variations allaient de légers troubles pigmentaires jusqu'à des aspects qui ressemblaient à s'y méprendre aux altérations caractéristiques du malade atteint de choroïdérémie.
Trois conductrices de trois arbres généalogiques différents présentèrent tant subjectivement qu'objectivement l'aspect de l'homme attient. Outres les plaintes d'héméralopie, ces femmes souffraient d'altérations du fond de l'oeil qui ressemblaient à celles d'un malade dans une phase avancée, ainsi que des altérations pathologiques de l'acuité visuelle, de l'adaptation à l'obscurité, du champ visuel, de l'ERG et de l'EOG. D'autre part, une conductrice, fille d'un homme atteint ne présenta aucune altération du fond de l'oeil.
Chez une femme on trouva un fond de l'oeil d'aspect exceptionnel, commeun“tapetum-likereflex”(reflettapétoïde), tel que Mann l'adécrit. Notre enquête nous a permis de confirmer l'hérédité intermédiaire liée au sexe.
Trois arbres généalogiques permirent de relever de la choroïdérémie combinés avec une autre affection de l'oeil liée au sexe, savoir deux fois avec la protanopie et une fois la deutéranopie. Il y avait lieu d'estimer le crossing-over comme extrêmement probable dans les trois arbres généalogiques.
Pour expliquer la présence, d'une part, du syndrome intégral de l'homme atteint chez trois hétérozygotes et l'absence, d'autre part, de toute altération quelle qu'elle soit chez une hétérozygote, on présume des changements dans l'expressivité du gène pathologique ou peut-être aussi de l'allèle.
Etant donné qu'aucune trace d'atrophie de la choroïde n'est observée dans la phase initiale, il semble probable que le siège de l'affection se trouve localisé dans la couche pigmentaire (tapetum nigrum). C'est la raison pour laquelle nous nous rallions à la proposition de Waardenburg de changer le nom de choroïdérémie en celui de dégénérescence progresssive tapéto-choroïdienne.
Atrophie gyrée de la choroïde et de la rétine
Les cinq arbres généalogiques ont permis d'identifier 13 malades dont 11 furent examinés. Les troubles les plus graves parurent avoir été causés par une myopie et une héméralopie survenant dans la première jeunesse.
Dans la phase initiale de l'affection on observa objectivement des zones atrophiées de la rétine et de la choroïde, nettement delimitées, dessinées en guirlande et situées dans la périphérie, zones qui s'etendent vers le pôle postérieur dans les phases ultérieures de la maladie allant alors souvent de pair avec une aire atrophiée péripapillaire et de forme annulaire.
Sur 11 cas de myopie un assez fort pourcentage progressait rapidement.
Chez un malade atteint d'hypermétropie, l'atrophie de la choroïde et de la rétine était localisée davantage vers le centre.
Notre enquête confirma également la fréquence de la cataracte de type compliqué chez les malades atteints d'atrophie gyrée. Nous avons constaté cette forme de cataracte chez 6 de nos 13 malades. Notre enquête permit de confirmer l'hérédité récessive autosomale de l'atrophie gyrée. La consanguinité constatée dans trois des cinq familles constitue un argument à l'appui de cette thèse. Chez une seule malade on constata une atrophie gyrée accompagnée d'une hétérochromie de Fuchs.
Il fut impossible de constater l'anomalie d'Alder chez six malades examinés à cet effet.
L'auteur accorde une attention particulière à la présence simultanée signalée par certains auteurs d'une dégénérescence tapéto-rétinienne et d'une atrophie gyrée de la choroïde et de la rétine, celle-ci n'ayant pu être confirmée par les cas qu'il a lui-même etudiés.
Zusammenfassung
Der erste Teil dieser Untersuchung bezieht sich auf das noch wenig bekannte Krankheitsbild der Chorioideremie, der zweite Teil auf die selten vorkommende Krankheit Atrophia gyrata chorioideae et retinae.
Nachdem in beiden Teilen die von anderen Autoren angetroffenen krankhaften Veränderungen und die Vererbung dieser hereditären Augenkrankheiten beschrieben worden sind, wird eine eigene Untersuchung von in Holland gefundenen Fällen besprochen; anschliessend werden unsere Befunde verglichen mit den Befunden anderer Autoren.
Chorioideremie
In den 9 Stammbäumen unter denen auch ein sehr grosser, durch Zusammenschluss von 4 anderen entstandener vorkam, wurden 52 befallene Männer und 96 Konduktorinnen gefunden, von denen 45 Befallene und 61 Konduktorinnen von uns untersucht werden konnten.
Befallene Männer
Als früheste Veränderungen des Augenhintergrundes wurden bei 6 jugendlichen Patienten, von denen der jüngste 1 1/2 Jahre alt war, nur feinkörnige Pigmentierungen im Aequator und in der Peripherie, und zwar in den tieferen Schichten der Netzhaut auf der Höhe des Pigmentepithels, gefunden. Wir konnten keine Spur von Chorioidealatrophie in diesem Anfangstadium beobachten. In den folgenden Stadien wurden Augenhintergrundsabweichungen festgestellt, welche leicht falsch gedeutet werden können, nämlich als Atrophia gyrata chorioideae et retinae bzw. als atypische tapetoretinale Degeneration mit Chorioidealsklerose.
Von 45 Befallenen gab es 28 mit Myopie, 12 mit Emmetropie, 4 mit Hypermetropie und 1 nicht bekannt. Emmetropie war hauptsächlich in den Anfangsstadien vorhanden. Bei 11 der 13 Patienten, deren Refraktion über längere Zeit verfolgt werden konnte, wurde während der Observation eine Progression der Myopie wahrgenommen.
Im Anfangsstadium wurden Dunkeladaptation, Gesichtsfelder, Elektroretinogramm und Elektro-okulogramm normal befunden. In späteren Stadien sahen wir pathologische Veränderungen auftreten.
Konduktorinnen
Bei Konduktorinnen wurden stark wechselnde Augenhintergrundsveränderungen angetroffen, welche schwankten zwischen leichten Pigmentstörungen und Bildern, die ziemlich gleichartig sind mit denen, welche für an Chorioideremie Leidende typisch sind.
3 Konduktorinnen aus 3 verschiedenen Stammbäumen zeigten subjektiv und objektiv das Bild eines befallenen Mannes. Ausser Beschwerden über Nachtblindheit, wurden bei diesen Heterozygoten Augenhintergrundsbilder ähnlich denen eines Befallenen in einem weiter entwickelten Stadium gefunden, sowie pathologische Veränderungen von Visus, Dunkeladaptation, Gesichtsfeld, ERG und EOG. Anderseits konnte man bei einer Konduktorin, Tochter eines befallenen Mannes, keine Veränderungen im Augenhintergrund feststellen. Ein merkwürdiges Fundusbild in Form eines “tapetum-like reflex”, wie Mann beschrieben hat, wurde bei einer Frau gesehen.
Die intermediär geschlechtsgebundene Vererbung der Chorioideremie konnte durch uns bestätigt werden.
In 3 Stammbäumen sahen wir eine Kombination von Chorioideremie mit einer anderen X-chromosomalen Augenkrankheit, nämlich zweimal mit Protanopia und einmal mit Deuteranopia. Crossing-over konnten wir in allen drei Stammbäumen mit grösster Wahrscheinlichkeit annehmen. Zur Erklärung des bei 3 Konduktorinnen beobachteten vollständigen Krankheitsbildes einerseits und des Fehlens von Veränderungen bei einer Konduktorin anderseits, werden Expressivitätsänderungen des pathologischen Gens oder möglicherweise auch des Allels angenommen. Auf Grund der Tatsache, dass im Anfangsstadium keine Spur von chorioidealer Atrophie beobachtet werden konnte, scheint es wahrscheinlich, dass der primäre Sitz der Erkrankung in der Pigmentschicht (tapetum nigrum) lokalisìert ist. Die durch Waardenburg vorgeschlagene Namensänderung der Chorioideremie in progressive tapeto-chorioideale Degeneration wird durch uns akzeptiert.
Atrophia gyrata chorioideae et retinae
In 5 Stammbäumen gab es 13 Patienten, von denen 11 untersucht wurden.
Die wichtigsten Beschwerden waren das Resultat einer im jugendlichen Alter auftretenden Myopie und Nachtblindheit.
Objektiv sah man im Anfangsstadium scharf begrenzte, girlandenförmige, in der Peripherie lokalisierte atrophische Bezirke der Aderhaut und Netzhaut, die sich in späteren Stadien in Richtung des hinteren Poles ausdehnten und oft mit einem ringförmigen peripapillären atrophischen Areal zusammenhingen.
In 11 Fällen wurde eine Myopie gefunden, wobei ein relativ grosser Prozentsatz Progression zeigte.
Bei einem hypermetropen Patienten war die Atrophie der Aderhaut und Netzhaut zentral lokalisiert.
Unsere Untersuchung bestätigte gleichzeitig das wiederholte Auftreten von Katarakta komplikata bei Patienten mit Atrophia gyrata. Wir fanden diese Form von Katarakt bei 6 unserer 13 Patienten. Die autosomale rezessive Vererbung der Atrophia gyrata konnte durch uns bestätigt werden. Auch die in 3 der 5 Familien gefundene Konsanguinität spricht dafür.
Kombination von Atrophia gyrata mit Heterochromie von Fuchs sahen wir bei einer Patientin.
Die Anomalie von Aider konnte bei 6 darauf untersuchten Patienten nicht nachgewiesen werden.
Spezielle Aufmerksamkeit wurde der von einigen Autoren gefundenen Kombination von Atrophia gyrata mit tapetoretinaler Degeneration gewidmet, was aber durch die vom Autor untersuchte Fällen nicht bestätigt werden konnte.
