Abstract

The time dependence of ischemia and reperfusion and the mechanism of ischemia precipitated pulsus alternans are not clear. In addition, the assessment and improvement of regional ventricular function need to be established. To investigate these, experiments were performed on open-chest anesthetized dogs. Central and peripheral arterial pressures, aortic flow, electrocardiogram and endocardial segment lengths in normal and ischemic zones were measured during 15 minutes of coronary artery ligation and during reperfusion. Results show different time courses of occlusion and reperfusion. Pressure-segment length loops are useful representations of regional ventricular function during control, digitalis, coronary artery ligation and propranolol, and parallel force-length relations in the isovolumic contraction and relaxation phases. The constant end-diastolic lengths in strong and weak beats indicate that the alternating contractile state is the responsible mechanism for the occurrence of pulsus alternans.
