Summary

In this paper we discuss three-dimensional reconstruction and flow measurements of the coronary artery vascular bed from multiple view coronary angiography. The reconstruction generates densitometric centerline and cross-sectional area measurements of the major branches of the arterial bed from two view ECG correlated digital angiograms for all images obtained throughout the cardiac cycle. The completeness of the reconstruction is well suited to allow computation of absolute flow through the major branches.

The arterial bed is represented as a branching tree structure. A projected structure including vessel branching and other recognizable node points is specificed on all images of both views. Dynamic programming with subsequent operator verification is used to determine the true vessel centerline and projected cross section in each image of each view. Each pair of computer determined structures is used to generate a three-dimensional representation of the arterial bed centerlines. The conversion factor between digital density value and vessel thickness is obtained from an average of all vessel profile measurements after correction for orientation and magnification. Segments which are too steeply inclined for orientation correction or which overlap other segments are not included in the averaging process. Absolute lumen area is then obtained from the densitometric profiles. Absolute flow measurements are obtained by tracking the leading edge of the iodine bolus down the known volume of the arterial tree.
