Somatische Morbidität psychisch Kranker

Zusammenfassung

Zwischen körperlichem und psychischem Krankheitsgeschehen bestehen vielfältige und komplexe Wechselbeziehungen. Bei schwer psychisch kranken Menschen, besonders solchen mit psychotischen und affektiven Störungen, sowie ausgeprägten Persönlichkeitsstörungen (»serious / severe mental illness« – SMI) ist im Vergleich zur Allgemeinbevölkerung sowohl ein erhöhtes Sterblichkeitsrisiko als auch ein gehäuftes Auftreten bestimmter somatischer Komorbiditäten nachweisbar. Als Erklärungsansätze für das erhöhte somatische Risiko psychisch Kranker werden unmittelbar und mittelbar krankheitsbezogene Faktoren, aber auch Faktoren auf Seiten des Gesundheitssystems diskutiert. Häufige und im psychiatrisch-psychotherapeutisch-psychosomatischen Kontext besonders zu berücksichtigende somatische Erkrankungen sind Herz-Kreislauf-, Lungen- und Atemwegserkrankungen, metabolische Erkrankungen, Karzinome, aber auch seltenere Störungen wie Bolusaspiration oder eine, insbesondere bei schizophren Erkrankten mitunter vorkommende, reduzierte Schmerzsensitivität.
