Zusammenfassung
Die Behandlung der chronischen, mit allen anderen Behandlungsmethoden nicht behandelbaren Schmerzen, der “intractable pains” erfolgt mit destruktiven und neuerdings mit stimulativen Methoden durch Ausschaltung der Schmerzleitungen im Gehirn (1, 25, 35, 38, 53). Dieser stereotaktische Operationstyp ist also die ultima ratio, wenn alle anderen Behandlungsmethoden in der Peripherie, d.h. unterhalb der Medulla oblongata versagt haben, oder wenn von vornherein feststeht, daß damit kein Resultat zu erzielen ist, wie z.B. beim Thalamussyndrom oder der Anaesthesia dolorosa nach Herpes zoster-Infektion. Die zahlreichen nationalen, kontinentalen und internationalen Schmerzkongresse und -symposien, der Zwang zur Organisation von Schmerzkliniken, die intensiven neurophysiologischen Untersuchungen auch im Hinblick auf Therapiekonzepte weisen darauf hin, welches Problem der Schmerz immer noch darstellt und hier im besonderen der chronische Schmerz. Was ist Schmerz? Der Schmerz ist zunächst ein subjektives Phänomen, er ist eine Bewußtseinserscheinung. Er existiert nur in Verbindung mit einem erfahrenden oder sensitiven Subjekt. Es gibt keinen unbewußten Schmerz. Während der Bewußtlosigkeit oder Narkose existiert kein Schmerz. Man könnte daher den Schmerz psychologisch abhandeln, man könnte phänomenologisch untersuchen, wie der Schmerz erlebt wird und was der Schmerz für das Subjekt bedeutet. Dabei kann man bis heute nicht beweisen, wenn jemand bestreitet Schmerzen zu haben, daß er tatsächlich keine Schmerzen hat und umgekehrt.
