Instability-Related Osteoarthritis: Special Indications for Osteotomies in the Treatment of the Unstable Knee

Abstract

An untreated rupture of the anterior cruciate ligament (ACL) leads to osteoarthritic changes in a large percentage of cases (Dejour etal. 1987; Jacobsen 1977; McDaniel and Dameron 1983). In 10- and 14-year follow-ups of untreated ACL ruptures, McDaniel and Dameron (1983) found an 84% incidence of meniscal lesions and an 86% incidence of osteoarthritic changes of varying degree, such as squaring of the medial femoral condyle, marginal osteophyte formation, joint-space narrowing, subchondral sclerosis, and peaking of the intercondylar eminence. An obvious relationship was noted between medial meniscectomy, joint-space narrowing with osteoarthritic changes, and varus deformity.
