Abstract
In eight patients with disseminated testicular cancer treated with cytostatic drug combinations including cisplatin (CP), we measured urinary N-acetyl-β-D-glucosaminidase (NAG) and gamma-glutamyl-transferase (γ-GT), and determined glomerular kidney functions. Patients were divided randomly into two groups. In one group we performed conventional nephroprotective infusion therapy; in a second group amino acids were added. After infusion of CP, the NAG activity increased dramatically and remained elevated for weeks. The γ-GT activity increased similarly but normalized within 2 days. Repeated CP exposure induced a cumulative increase in enzymuria. Variation in glomerular function did not substantially influence increased enzyme leakage. Osmodiuretic agents improved in part the glomerular filtration rate; however, renal enzyme loss remained elevated. Amino acid-enriched infusions resulted in a marked reduction, but not normalization of urinary NAG leakage. It is concluded that 1. urinary NAG activity monitors CP-related autophagia in lysosomes of renal tubular cells and 2. serves as a useful parameter for measuring the individual state of CP-induced nephrotoxicity and 3. amino acids exhibit a marked suppression of CP-related nephrotoxicity in proximal tubules of the kidney.
Zusammenfassung
Bei acht Patienten mit NSGCT unter Polychemotherapie nach dem modifizierten Einhorn-Schema (PVB) haben wir die Aktivität des lysosomalen Enzyms N-Aeetyl-β-D-Glucosaminidase und des “brushborder”-Enzymes Gamma-Glutamyl-Transferase (γ-GT) im Urin gemessen sowie glomeruläre Funktionen der Niere untersucht. Zur Nephroprotektion verwendeten wir a) eine konventionelle und b) eine variierte, mit Aminosäuren angereicherte Form der Infusionstherapie. Nach Infusion von Cisplastin stieg die NAG-Aktivität im Urin stark an und blieb über mehrere Wochen erhöht. Wiederholte Cisplatin-Applikationen (Zyklen) ergaben einen kumulativen Effekt. Die glomeruläre Filtrationsrate zeigte keinen substantiellen Einfluß auf die Enzymleckraten von NAG und γ-GT. Osmodiuretika förderten die glomeruläre Funktion, ohne gleichzeitig die renalen Enzymlecks zu vermindern. Dem gegenüber trat unter Infusion von Aminosäuren eine drastische Suppression, jedoch keine völlige Normalisierung des NAG-Lecks auf. Wir schlußfolgern, daß 1. die erhöhte NAG-Aktivität im Urin Ausdruck einer Steigerung der lysosomalen Autophagic nach Cisplatin-Applikation ist, 2. die NAG-Enzymurie einen empfindlichen Parameter für die Verlaufskontrolle der Cisplatin-induzierten Nephrotoxizität darstellt und 3. Aminosäureinfusionen einen ausgeprägten suppressiven Effekt auf die am proximalen Tubulus angreifende Nephrotoxizität von Cisplatin haben.
