Zusammenfassung
Eine überhöhte sympathische Aktivität bedingt eine überhöhte Druck- und Volumenbelastung des Herz-Kreislaufsystems und einen überhöhten myokardialen Sauerstoffbedarf. Verschiedene Arbeitskreise beschäftigen sich seit Jahren mit der Frage eines veränderten sympathischen Tonus oder einer gestörten Empfindlichkeit des Organismus gegenüber Katecholaminen bei der Koronaren Herzkrankheit und Koronarinsuffizienz. Entscheidende Determinanten des myokardialen Sauerstoffbedarfs, wie Vorlast, Nachlast, Herzfrequenz und Kontraktilität des Herzens stehen unter dem Einfluß des Sympathikus (Abb. 1). Die freien Plasma-, weniger Harnkatecholamine, Noradrenalin und Adrenalin gelten als biochemische Bruttoindikatoren der sympathischen Aktivität (27, 28).
