Zusammenfassung
Die Pathogenese der Sepsis und des septischen Schocks ist trotz zahlreicher Untersuchungen immer noch Gegenstand kontroverser Diskussionen. Zweifellos kommt jedoch den Leukozyten, speziell den polymorphkernigen (PMN) Granulozyten, eine hervorragende Rolle in der Ausbildung des sepsisbedingten multiplen Organversagens zu. Die Einwanderung von PMN-Granulozyten in das Entzündungsgebiet wird u.a. durch die Bildung von leukotaktischen Substanzen wie Leukotrienen, Fibrin/ogen/spaltprodukten, Anaphylatoxinen aus dem Komplementsystem (C5a, C3a) etc. sowie durch eine zunehmende Gefäßpermeabilität aufgrund der gesteigerten Wirkung von Kininen und Arachidonsäuremetaboliten ausgelöst bzw. aufrechterhalten. Während der Bindung und Aufnahme von invasiven Organismen oder Zellbruchstücken im Infektionsherd setzen die Granulozyten zahlreiche aggressive Substanzen frei (Sauerstoffradikale, hydrolytische und proteolytische Enzyme etc.), die dann im Extrazellulärraum Strukturelemente (Basalmembranen, Elastin, Kollagen, Fibronektin, Proteoglykane u.a.) ebenso wie humorale Faktoren (insbesondere Proteine der Kaskadensysteme: Gerinnung, Fibrinolyse, Komplement und Kallikrein-Kinin-System) nachhaltig schädigen können (Literaturübersicht bei [10]).
