Zusammenfassung
Die Entwicklung computergestützter Operationshilfen hat sich zwangsläufig aus den heutigen Möglichkeiten dreidimensionaler Schnittbildverfahren, rapider Datenverarbeitung und Datenübertragung sowie digitaler Bildverarbeitung ergeben. Die konventionelle Chirurgie kann mit handgeführten Instrumenten naturgemäß nicht die hohe geometrische Genauigkeit diagnostischer Bildinformationen und der daraus resultierenden präoperativen Planung umsetzen. Diese große Diskrepanz, auch zwischen Operateuren mit unterschiedlicher Erfahrung, läßt sich nur durch den Einsatz programierbarer aktiver Werkzeuge zum Wohle des Patienten vermindern. Wie schon lange aus der industriellen Fertigung bekannt, können mit Hilfe des Roboters präoperativ gewonnene individuelle digitale Planungsergebnisse in beliebig reproduzierbarer Form exakt umgesetzt werden. Voraussetzung für dieses Verfahren ist in der Regel die rigide Fixierung des zu bearbeitenden Materials und eine hochpräzise Registrierung. Das von orto MAQUET entwickelte universelle Operationssystem CASPAR erfüllt diese Kriterien in idealer Weise für die Knochen- und Gelenkchirurgie. Das CASPAR-System wird klinisch in Europa seit 1997 erfolgreich zur Planung und Ausführung der Hüftschaftimplantation eingesetzt. Der klinische Einsatz beim Kreuzbandersatz ist seit April 1999 möglich.
