Zusammenfassung
Für die Beurteilung der Spätergebnisse nach Magenresektion oder Vagotomie sind neben den Ulkusrezidivraten die Folgesymptome von Bedeutung. Dazu wird unter anderem die von Visick beschriebene Klassifikation herangezogen. Nach Magenresektion ist die Rekonstruktion nach Billroth II (oder Roux-YGastrojejunostomie) funktionell eindeutig weniger günstig als diejenige nach Billroth I. Dumpingbeschwerden, Diarrhö, Gewichtsverlust, Anämie und Osteoporose sind in höherem Prozentsatz nach Gastrojejunostomie als nach Gastroduodenostomie anzutreffen. Das Magenstumpfkarzinom entsteht aus der langjährig persistierenden atrophen Gastritis mit intestinaler Metaplasie, die als fakultative Präkanzerose anzusehen ist.
Alle Vagotomieformen verändern die Magenmotorik und die rezeptive Relaxation des Fundus. Nach Denervation fehlt die koordinierte Schrittmacherfunktion des Magens. Postoperative Folgezustände betreffen Dysphagie, Dumping und Diarrhö. Nur nach proximal selektiver gastrischer Vagotomie mit Erhaltung der Antruminnervation sind diese Folgezustände minimal ausgeprägt.
