Abstract
Irregular sleep-wake rhythm disorder (ISWRD) is characterized by a dysrhythmic sleep-wake pattern with multiple cycles across the 24-hour day. It is a rare condition that occurs predominantly among children with neurodevelopmental disorders, older adults with neurodegenerative diseases, patients with traumatic brain injury, and some individuals with schizophrenia. The exact pathophysiology of ISWRD is not known, but weak zeitgeber input, abnormal functioning of the endogenous circadian clock, and irregular behavioral patterns are all thought to play a role. Although few evidence-based treatments exist, morning bright light exposure improves behavioral symptoms such as nocturnal wandering and restlessness among elderly patients with dementia and exogenous melatonin improves select sleep parameters among children with neurodevelopmental disorders.
