Abstract
The common epithelial tumors of the ovary constitute about two-thirds of all primary ovarian neoplasms and almost 90% of all malignant ovarian tumors.151 They are considered to be derived from the surface epithelium (coelomic epithelium or mesothelium) covering the ovary and from the underlying stroma.157a Although this histogenesis has been accepted by most authorities, the supporting evidence varies in the individual tumors from fairly conclusive to circumstantial. The histologic heterogeneity encountered in this group of tumors does not contradict their common origin. Although the ovary is not of müllerian origin, the source of these neoplasms, namely, the surface epithelium, is derived from the coelomic epithelium, which in the embryo gives rise to the müllerian ducts. The latter form the fallopian tubes, uterine body, cervix, and possibly the upper part of the vagina with their large variety of epithelia. The presence of different epithelial elements in these tumors can, therefore, be satisfactorily explained. Furthermore, admixtures of histologic elements from various epithelial ovarian tumors within a given tumor are very common and constitute further evidence of their common derivation. The cause for the great neoplastic potential of the ovarian surface epithelium, however, has not yet been elucidated.
